摘要
目的探讨纤维支气管镜吸痰联合肺泡灌洗在重症肺部感染患者中的应用效果及护理措施,给予临床诊治指导。方法选取2012年1月-2014年1月重症肺部感染患者90例,按数学随机方法分为普通组和治疗组,各45例,普通组实施内科常规治疗,治疗组实施纤维支气管镜吸痰联合肺泡灌洗,对比两组患者治疗有效率、血氧分压等临床指标。结果普通组治疗有效率为62.22%,治疗组有效率为82.22%,两组对比差异有统计学意义(χ2=9.218,P<0.05);治疗组血氧分压(PaO2)为(81.44±0.84)mm Hg、二氧化碳分压(PaCO2)为(42.28±0.92)mm Hg、血氧饱和度(SaO2)为(91.88±1.34)%、pH值为7.38±0.06,各项指标改善均优于普通组,对比差异有统计学意义。结论在重症肺部感染患者中应用纤维支气管镜吸痰及灌洗治疗临床效果显著确切,安全性高,值得临床推广应用。
OBJECTIVE To observe the effect of bronchoscope sputum suction combined with bronchoalveolar lav -age on treatment of the patients with severe pulmonary infections and put forward the nursing measures so as to provide guidance for the clinical diagnosis and treatment .METHODS From Jan 2012 to Jan 2014 ,a total of 90 pa-tients with severe pulmonary infections were enrolled in the study and randomly divided into the common group and the treatment group ,with 45 cases in each ;the common group was treated with conventional internal medicine therapy ,while the treatment group was given the bronchoscope sputum suction combined with bronchoalveolar lavage ;the clinical indicators such as the effective rate of treatment and partial pressure of blood oxygen were ob -served and compared between the two groups .RESULTS The effective rate of treatment was 62 .22% in the com-mon group ,82 .22% in the treatment group ,there was significant difference between the two groups （χ2 = 9 .218 , P〈 0 .05） .The partial pressure of arterial oxygen （PaO2 ） was （81 .44 ± 0 .84）mmHg ,the partial pressure of ar-terial carbon dioxide （PaCO2 ）（42 .28 ± 0 .92）mmHg ,the arterial oxygen saturation（SaO2 ）（91 .88 ± 1 .34）% ,the pH 7 .38 ± 0 .06 ,the improvements of the indicators of the treatment group were better than those of the common group ,and there was significant difference .CONCLUSION The bronchoscope sputum suction combined with bron-choalveolar lavage can achieve significant ,exact clinical effect on treatment of the patients with severe pulmonary infections ,with the safety high ,and it is worthy to be promoted in the hospital .
