摘要
目的：观察电针合谷三阴交对硬膜外神经阻滞分娩镇痛的影响。方法：足月妊娠待产妇80例随机分为电针组和对照组（n=40）。患者宫缩进入活跃期后行L3-4椎间隙硬膜外穿刺置管,接便携式PCA装置进行常规镇痛。电针组实施合谷、三阴交穴位电刺激。观察产妇的生命体征、下肢运动神经阻滞评分（Bromage评分）、产程持续时间、分娩方式、催产素使用率、局麻药物使用量、新生儿Apgar评分。结果：两组患者MBP、HR、VAS在镇痛后均有下降（P﹤0.05）,组间比较差异无统计学意义（P〉0.05）。E组Bromage评分在第二三产程末高于C组（P﹤0.05）。与C组比较,E组第一、二产程持续时间缩短（P﹤0.05）,正常阴道分娩率明显多于C组（P﹤0.05）,催产素使用人数及局麻药使用量均小于C组（P﹤0.05）;新生儿Apgar评分组间比较,差异无统计学意义（P〉0.05）。结论：电针合谷、三阴交可以减少硬膜外神经阻滞无痛分娩的一些不良反应,减少局麻药用量,减少运动神经阻滞,减少缩宫素使用,加快产程,促进正常分娩,对新生儿无明显影响。
Objective： To observe the effects of electroacupuncture at Hegu and Sanyinjiao on epidural nerve block for labor analgesia. Methods： 80 cases of full- term pregnancy expectant mothers were randomly divided into the electroacupuncture group（ E group） and the control group（ C group）（ n = 40）. Patients in the active phase of uterine contraction underwent L3- 4intervertebral epidural catheterization,and then a portable device for regular PCA analgesia. EA group implemented electrical stimulation at Hegu and Sanyinjiao. The vital signs,lower limb motor block score（ Bromage score）,duration of labor,mode of delivery,oxytocin usage,the amount of drug used for local anesthesia of the patients and Apgar score of newborn were observed. Results： MBP,HR and VAS of the patients in two groups were decreased after analgesia（ P〈 0. 05） and there was no statistically significant difference between groups（ P〈 0. 05）. Bromage scores of group E were better at the end of the second and third stage of labor than that of group C（ P〈 0. 05）. Compared with group C,the durations of the first and second stage were shorter than that in group E（ P〈 0. 05） and there were more normal vaginal deliveries than that of group C（ P〈 0. 05） and the number of patients using oxytocin and the amount of local anesthetics were less in group E than those in group C（ P〈 0. 05）. The difference of Apgar score between two groups was not statistically significant（ P〈 0. 05）. Conclusions： The electroacupunture at Hegu and Sanyinjiao can reduce some side effects of epidural nerve block for labor analgesia,reduce the amount of local anesthetic,reduce motor block,reduce the usage of oxytocin,speed up the labor process and promote Vaginal delivery. The newborn had no significant effect.
