溶血葡萄球菌对8种抗菌药物及诱导克林霉素耐药分析 被引量:2

Analysis of staphylococcus haemolyticus on 8 kinds of antibiotics and inducible clindamycin resistance

摘要 目的分析溶血葡萄球菌的抗药性,及红霉素对克林霉素诱导性耐药的发生率,为临床治疗提供依据,使临床更加合理有效地使用抗菌药物,提高治疗成功率。方法分离培养各类标本中的溶血葡萄球菌,琼脂稀释法检测8种常用抗菌药物的耐药性,并做纸片法D试验检测红霉素诱导克林霉素耐药发生率。用头孢西丁纸片法检测耐甲氧西林溶血葡萄球菌(methicillin-resistant Staphylococcus haemolyticus,MRSH)。结果 125株溶血葡萄球菌中MRSH 115株,占92.0%;甲氧西林敏感溶血葡萄球菌(methicillin-susceptible Staphylococcus haemolyticus,MSSH)10株,占8.0%。总D试验阳性35株,占28.0%;D试验阴性90珠,占72.0%。对环丙沙星、万古霉素、红霉素、左氧氟沙星、青霉素、头孢呋辛、庆大及克林霉素的耐药性分别为81.0%、0.0%、88.0%、51.5%、100.0%、59.1%、54.5%、76.0%。结论溶血葡萄球菌对红霉素、克林霉素等常用药物耐药严重,常规D试验检测有助于临床合理使用大环内酯类及林可霉素类抗菌药物。 Objective Analysis of drug resistance of staphylococcus haemolyticus,incidence of inducible clindamycin and erythromycin resistance,to provide basis for clinical treatment,more rational use of antimicrobial drugs effectively in clinics,improve the success rate of treatment. Methods In specimens of staphylococcus hemolysis,detection of agar dilution method of 8 kinds of antimicrobial drug resistance,and method of D test for the detection of erythromycin incidence of inducible clindamycin resistance. By cefoxitin disc diffusion method for detection of MRSH. Results 115 strains of MRSH in 125 staphylococcus hemolytic,accounted for92. 0%,10 strains of MSSH,accounted for 8. 0%. Total D test positive strains 35,accounted for 28. 0%,D test negative 90 strains,accounted for 72. 0%. Resistance to ciprofloxacin,vancomycin,erythromycin,levofloxacin,penicillin,cefuroxime,gentamycin and clindamycin were 81. 0%,0. 0%,88. 0%,51. 5%,100. 0%,59. 1%,54. 5%,and 76. 0% respectively. Conclusions Staphylococcus haemolyticus have serious resistance to commonly used drugs like erythromycin and clindamycin,the conventional D test is helpful to the rational clinical use of macrolides and lincomycin antibiotics.
