摘要
目的:探讨microRNA-21(miR-21)与冠心病患者冠状动脉(冠脉)病变严重程度的相关性。方法:收集2011-06-2013-06在昆明医科大学第二附属医院心内科住院病例121例,根据临床诊断分为胸痛综合征(CPS)组32例,稳定型心绞痛(SAP)组26例,不稳定型心绞痛(UAP)组41例,急性心肌梗死(AMI)组22例。所有病例均行冠脉造影检查,根据冠脉造影结果对冠脉病变进行Gensini评分。采用Real-Time PCR法检测外周血miR-21的表达水平。结果:1冠心病组的miR-21水平明显高于非冠心病组。2AMI组及UAP组miR-21水平较CPS组明显升高,而SAP组与CPS组无明显差异。3冠脉3支病变及2支病变患者的miR-21水平较冠脉无病变及1支病变患者明显升高。4miR-21水平与Gensini评分呈正相关(r=0.746,P<0.001)。结论:外周血miR-21水平与冠心病患者冠脉病变严重程度呈正相关,为临床诊断冠心病和预测冠脉病变提供了新的依据。
Objective:To investigate the correlationship between microRNA-21(miR-21)and severity of coronary stenotic lesions in coronary heart disease patients.Methods:All 121 subjects,including patients with acute myocardial infarction(AMI,n=22),unstable angina pectoris(UAP,n=41),stable angina pectoris(SAP,n=26)and chest pain syndrome(CPS,n=32),enrolled in this study.All patients underwent coronary angiography.Gensini score was calculated according to the results of coronary angiography.The expression levels of peripheral blood miR-21 were detected by using Real-Time PCR method.Result:1miR-21 levels in the group of coronary heart disease(CHD)were significantly higher than that in non-CHD group.2miR-21 levels in AMI group and UAP group were significantly higher than those in CPS group,but there was no significant difference in SAP group and CPS group.3miR-21 levels in the subjects with diseased vessels of 2and≥3were significantly higher than those with diseased vessel of 0and 1.4miR-21 levels were positive associated with Gensini scores(r=0.746,P〈0.001).Conclusion:miR-21 expression is positive associated with the severity of coronary stenotic lesions calculated by Gensini scores,supporting a new basis for the clinical diagnosis of coronary heart disease and the predictor of coronary lesions.
