婴幼儿毛细支气管炎心肌酶谱及高敏肌钙蛋白T变化的临床分析 被引量:5

摘要 目的 探讨婴幼儿毛细支气管炎心肌酶谱及高敏肌钙蛋白T变化的临床意义。方法 随机选取2012年3月~2014年3月在河北省昌黎县人民医院儿科和河北省昌黎县妇幼保健院儿科住院治疗的毛细支气管炎患儿78例(观察组),根据年龄分为两组,观察组1 40例,年龄≤6个月;观察组2 38例,年龄〉6~24个月。选取同期在河北省昌黎县妇幼保健院儿保门诊体检的正常婴幼儿75例作为对照组。三组分别检测并比较心肌酶谱及高敏肌钙蛋白T(hs-c Tn T)水平。结果 观察组78例,心肌酶谱中冬氨酸氨基转移酶(AST)异常发生率为62.8%,乳酸脱氢酶(LDH)异常发生率为71.8%,a-羟丁酸脱氢酶(a-HBDH)异常发生率为67.9%,肌酸激酶(CK)异常发生率为57.7%,肌酸激酶同工酶(CK-MB)异常发生率为51.3%,hs-c Tn T异常发生率为74.4%。对照组心肌酶谱(AST、LDH、a-HBDH、CK、CK-MB)及hs-c Tn T异常率分别为5.3%、10.7%、8.0%、2.7%、2.7%及1.3%,两组比较差异有统计学意义(P〈0.05),提示毛细支气管炎多数有心肌酶谱、hs-c Tn T改变及心肌损害。观察组1 AST为(94.2±20.9)U/L,LDH为(458.4±86.3)U/L、a-HBDH为(352.8±69.6)U/L,CK为(438.4±94.8)U/L,CK-MB为(70.3±15.8)U/L,hs-c Tn T为(0.035±0.006)ng/m L;观察组2 AST为(58.4±18.6)U/L,LDH为(352.6±82.7)U/L、a-HBDH为(261.4±82.5)U/L、CK为(346.9±98.6)U/L、CK-MB为(48.6±17.2)U/L、hs-c Tn T为(0.020±0.003)ng/m L;观察组1患儿心肌酶谱及hs-c Tn T变化大于观察组2,差异均有统计学意义(均P〈0.05),提示〈6个月毛细支气管炎患儿更容易出现心肌酶谱、hs-c Tn T改变及心肌损害。结论 毛细支气管炎患儿心肌酶谱、hs-c Tn T异常的发生率较高,及时检测心肌酶谱及hs-c Tn T有助于临床综合分析、判断病情和有效治疗。 Objective To study the clinical significance of myocardial enzymes and high-sensitivity cardiac troponin T changes in infants with capillary bronchitis. Methods 78 cases of infants with capillary bronchitis treated in People's Hospital of Changli County and Maternity and Child Care Centers of Changli County from March 2012 to March 2014 were selected as observation group. According to the age, they were divided into the two groups. Observation group 1had 40 cases, aged ≤6 months, observation group 2 had 38 cases, aged6-24 months. 75 normal infants at the same period taken physical examination in Pediatric Clinic of Maternity and Child Care Centers of Changli County were selected as control group. The myocardial enzymes and high-sensitivity cardiac troponin T(hs-c Tn T) levels were detected and compared among the groups. Results In observation group, the incidence of abnormal myocardial enzyme [aspartate aminotransferase(AST), lactate dehydrogenase(LDH), a-hydroxybutyrate dehydrogenase(a-HBDH), creatine kinase(CK), creatine kinase isoenzyme(CK-MB)] were 62.8%, 71.8%, 67.9%, 57.7%, 51.3% respectively, while the incidence of abnormal hs-c Tn T was 74.4%; in the control group, the incidence of abnormal myocardial enzyme(AST,LDH, a-HBDH, CK, CK-MB) were 5.3%, 10.7%, 8.0%, 2.7%, 2.7% respectively, while the incidence of abnormal hsc Tn T was 1.3%; which had significant differences between the two groups(P〈0.05), suggested the capillary bronchitis had myocardial enzymes, hs-c Tn T changes and myocardial damage. In observation group 1, the levels of AST, LDH, aHBDH, CK, CK-MB and hs-c Tn T were(94.2±20.9) U/L,(458.4±86.3) U/L,(352.8±69.6) U/L,(438.4±94.8) U/L,(70.3±15.8) U/L and(0.035±0.006) μg/L, which in observation group 2 were(58.4±18.6) U/L,(352.6±82.7)U/L,(261.4±82.5)U/L,(346.9±98.6) U/L,(48.6±17.2) U/L and(0.020±0.003) μg/L, the differences were statistically significant(P〈0.05), suggested that infants with capillary bronchitis aged 6 months were more likely to suffer from myocardial enzymes, hs-c Tn T changes and myocardial damage. Conclusion The incidence of abnormal myocardial enzymes and hsc Tn T is higher in infants with capillary bronchitis, timely detection of myocardial enzymes and hs-c Tn T can help for clinical comprehensive analysis, determine the illness and effective treatment.
