摘要
目的探讨乙醛脱氢酶1(ALDH1)在人胃癌组织中的表达与幽门螺杆菌L型(Hp-L型)感染的关系。方法应用免疫组化Elivision法和原位杂交方法检测126例胃癌及30例正常对照胃组织ALDH1的表达,应用革兰染色和免疫组化法检测上述组织的Hp-L型感染情况。结果胃癌组织中ALDH1蛋白和mRNA的表达阳性率分别为53.2%和56.3%,与对照组之间差异具有统计学意义(均P<0.01),ALDH1蛋白的表达与肿瘤的临床TNM分期和淋巴结转移密切相关(均P<0.01)。胃癌及对照组中Hp-L型的阳性率分别为67.5%和23.3%,差异具有统计学意义(P<0.01)。胃癌组Hp-L型感染阳性者的ALDH1蛋白表达阳性率高于Hp-L型感染阴性组(P<0.05)。ALDH1蛋白的表达与Hp-L型感染呈正相关(rs=0.220,P<0.05)。ALDH1阳性患者5年生存率明显低于ALDH1阴性患者(P<0.05)。结论 ALDH1在胃癌组织中高表达,可作为评价胃癌预后的指标,Hp-L型感染可促进其表达。
Objective To investigate the relationship between ALDH1 expression and Helicobacter pylori L（Hp‐L）‐form infection in gastric carcinoma.Methods In 126 cases with gastric carcinoma and 30 healthy individuals ,expression of ALDH1 protein and mRNA was examined by immunohistochemical staining with Elivision method and in situ hybridization ,respective‐ly.In addition to the above tissues ,Hp‐Lwas detected by means of Gram stain and immunohistochemical staining with Elivision method.Results It was found that in gastric carcinoma ,the positive rate of ALDH1 protein and mRNA was 53.2% and 56.3% ,respectively ,significantly higher than that of normal controls（P〈 0.01）.The expression of protein was significantly correlated with TNM stage and lymph node metastasis（P〈0.01）.In these gastric carcinoma cases ,the positive rate for Hp‐L （67.5 % ）was higher than that in the normal controls（23.3% ）（P〈0.01）.ALDH1 protein expression was higher in the Hp‐L positive cases than in the H p‐L negative cases（P〈0.05）. There was positive correlation between ALDH1 expression and posi‐tive Hp‐L staining（rs =0.220 ,P〈0.05）.Kaplan‐Meier survival analysis showed that the patients with positive expression of ALDH1 had lower 5‐year overall survival rate than those with negative expression（P〈0.05）.Conclusion There is higher ex‐pression of ALDH1 in gastric carcinoma. It could serve as an important factor for the prognosis of gastric carcino‐ma. Helicobacter pylori L‐form may be associated with the up‐regulation of ALDH1.
