培哚普利对冠心病伴舒张功能不全患者血浆NT-proBNP及组织多普勒指标的影响 被引量:2

The effect of perindopril on plasma amino-terminal b-type brain natriuretic peptide and tissue doppler indexes in the patients of coronary artery disease with diastolic dysfunction

摘要 目的探讨培哚普利对冠心病伴舒张功能不全患者血浆氨基末端脑钠肽前体(NT-proBNP)及组织多普勒指标的影响。方法经冠状动脉造影诊断为冠心病伴舒张功能不全的患者120例,分为普利组和对照组各60例,两组均予冠心病常规治疗,普利组患者在冠心病常规治疗基础上给予培哚普利4~8 mg/d口服治疗6个月。所有患者用药前后分别测量血浆NTproBNP的浓度、左室射血分数(EF)、舒张早期二尖瓣口血流速度(E)、舒张晚期二尖瓣口血流速度(A)、舒张早期二尖瓣环运动速度(Em),并计算E/A、E/Em的值。结果 NT-proBNP与E/Em相关(r=0.79,P〈0.01)。治疗6个月后,普利组患者血浆NT-proBNP水平明显降低(t=11.97,P〈0.001),对照组患者血浆NT-proBNP水平也降低(t=8.86,P〈0.001);与对照组比较,治疗6个月后普利组患者血浆NT-proBNP水平降低明显(t=3.56,P〈0.001)。治疗6个月后,普利组患者E/Em值较前降低(t=3.26,P=0.023),Em值均较前升高(t=-4.06,P〈0.001);与对照组相比,普利组治疗6个月后,E/Em值降低明显(t=10.46,P〈0.001),Em值升高明显(t=6.72,P〈0.001)。结论培哚普利对冠心病患者左室舒张功能有明显改善作用。 【Objective】To study the effect of perindopril on plasma amino-terminal B-type brain natriuretic peptide(NTproBNP) and tissue Doppler indexes in the patient of coronary artery disease with diastolic dysfunction. 【Methods】Selected 120 patients diagnosed with coronary heart disease with diastolic dysfunction by coronary angiography and Doppler imaging, Which were randomly divided into treatment group(60 cases) and control group(60 cases), Two groups of patients were treated with coronary heart disease conventional treatment, the patients of treatment group were treated with perindopril 4-8 mg/d on the basis of routine treatment, medication for six months, Compare the levels of NT-proBNP, early diastolic mitral valve flow velocity(E),late diastolic mitral valve flow velocity(A),early diastolic velocity mitral annulus velocity(Em),E/A,E/Em and left ventricular ejection fraction before and after six months treatment and made comparison. 【Results】NT-proBNP relate to E/Em(r=0.79,P〈0.01).After six months treatment, The plasma NT-proBNP of patients in the treatment group decreased significantly(t=11.97,P〈0.001),The plasma NT-proBNP of patients in the control group decreased(r=8.86,P〈0.001);Compared with the control group, After six months treatment, The plasma NT-proBNP of patients in the treatment group decreased significantly(t=3.56,P〈0.001).After six months treatment, E/Em in the treatment group decreases(t=3.26,P=0.023), Em increases(t=-4.06,P〈0.001); Compared with the control group, After six months treatment, E/Em in the treatment group decreases significantly(t=10.46,P〈0.001), Em increases significantly(t=6.72,P〈0.001).【Conclusion】Perindopril can improve the diastolic function of the patients of coronary artery disease with diastolic dysfunction.
