老年肺硬化性血管瘤的诊治分析(附86例报告) 被引量：1

摘要 目的总结老年肺硬化性血管瘤(PSH)的临床表现、病理学特征、影像学特点及鉴别诊断。方法回顾性分析86例老年PSH患者的临床病理资料,对其临床特征、影像学检查结果和病理诊断特点等进行分析。结果老年PSH多见于女性,常无明显临床症状或表现为咳嗽、咳痰等轻微的非特异性症状。影像学上均表现为圆形或类圆形肺内孤立性结节,但均未见有新月征。病理学检查示老年PSH由圆形间质细胞和表面细胞构成,镜下可分为乳头状结构、实性结构、血管瘤样区和硬化性结构。免疫组化检查以TTF-1、EMA、AE1/3和Syn阳性表达为主。结论老年PSH临床特征不典型,易于误诊;胸部CT扫描虽有助于鉴别与诊断,但最终确诊需依靠组织病理学,当无法确诊时,首选手术探查切除且预后良好。
