摘要
目的:探讨3D腹腔镜系统在外科手术中的应用。方法:选取2013年11月至2014年2月应用3D腹腔镜系统完成腹腔镜手术84例,包括单孔胆囊切除术12例、单孔阑尾切除术15例、直肠癌根治术5例、右半结肠癌根治术4例、肺癌根治术4例、肺叶切除4例、肾上腺肿瘤切除术6例、肾囊肿去顶减压术2例、肾部分切除术4例、肾癌根治术3例、广泛性全子宫切除术25例,统计手术时间、出血量、术后住院时间等数据。结果:84例手术均获得成功,手术平均时间较2D腹腔镜系统下缩短30-70 min;术中出血30-350 m L;术后住院时间2-7 d,平均5 d。结论:3D腹腔镜系统采用电荷耦合元件(charge coupled device,CCD)前置技术,还原了真实视觉中的三维立体手术视野,使不同层面的血管、神经和组织间隙之间的立体感更强,解剖位置更为明确,从而降低了手术难度,缩短了手术时间。该系统具备腹腔镜与开放手术的共同优势,使手术解剖更精确,缝合相对容易,且手术安全,疗效显著,并无明显并发症发生。
Objective To investigate 3D imaging laparoscopy system in surgery. Methods Totally 84 cases of 3D laparoscopic operation from November 2013 to February 2014 were enrolled in the study, including 12 ones of single port cholecystectomy, 15 ones of single port appendectomy, 5 ones of radical resection of rectal carcinoma, 4 ones of radical resection of right colon carcinoma, 4 ones of radical resection of lung cancer, 4 ones of pulmonary lobectomy, 6 ones of adrenal tumor resection, 2 ones of renal cyst decortication, 4 ones of heminephrectomy, 3 ones of radical nephrectomy of renal carcinoma and 25 ones of complete hysterectomy. The data on operating time, blood loss, post-operation hospital stay and etc were summarized. Results All the 84 cases were successfully completed, with the operating time less than that with 2D laparoscope by 30 min to 70 min, transoperative blood loss from 30 m L to 350 m L, and post-operation hospital stay from 2 d to 7 d and the mean one of 5 d. Conclusion 3D laparoscope with preset charge coupled device(CCD)can show clearly 3D visual operative field, vessels, nerves, tissues and anatomical position, and thus decrease the operation difficulty and time. 3D laparoscopy system gains the advantages of laparoscopic and open surgeries, with precision dissection, easy saturation and few complications.
