摘要
目的探讨应用隧道式瘘管剔除结扎术治疗单纯性高位肛瘘的临床疗效和安全性。方法前瞻性将2012年7月至2013年6月期间收治的符合纳入标准的180例单纯性高位肛瘘患者按随机序列表法分成两组,每组90例,分别行隧道式瘘管剔除结扎术(治疗组)和肛瘘切缝挂线引流术(对照组)。观察两组的根治率、复发率、术后肛门功能、术后疼痛、愈合时间。结果治疗组与对照组在根治率、复发率差异无统计学意义,两组在保护肛门功能、平均愈合时间、术后疼痛差异有统计学意义。结论隧道式瘘管剔除结扎术疗效好、疗程短、复发率低、对控便功能损伤小,是高位单纯性肛瘘治疗的可行选择。
Objective To evaluate the clinical efficacy and safety of tunnel culling and ligation in the treatment of simple high anal fistula.Methods One hundred eighty-six students cases of high anal fistula from the anorectal department of our hospital and Chengdu anorectal Hospital during 2012 July to 2013 June were divided into two groups,with 90 cases in treatment group which will be treated with tunnel culling and ligation and 90 cases in control group which will be treated with incision and sutureseton drainage.The cure rate,recurrence rate,postoperative anal function,postoperative pain,healing time of the two groups were observed.Results There was no significant difference of incure rate and recurrence rate in the two group,but there were significant differences of the protection of anal function,the average healing time, and the postoperative pain between the two groups.Conclusion Tunnel culling and ligation has good efficacy,shorter healing time,lower recurrence rate and smaller damage of anal function,which is a feasible choice for high simple anal fistula.
