摘要
目的探讨人工桡骨头置换术治疗肘关节不稳定的桡骨粉碎性骨折的临床疗效。方法对新鲜闭合性MasonⅢ型及以上桡骨头患者33例进行研究,分为三组,分别采用桡骨头切除术、内固定术及人工桡骨头置换术治疗。观察三组术后肘关节功能及近期、远期并发症。结果人工桡骨头置换术组Broberg和Morrey评分、提携角、桡骨近端移位程度及前臂旋转均优于其它方法,差异具有统计学意义(P<0.05)。并发症对比,三组间无统计学差异(P>0.05)。结论人工桡骨头置换术治疗肘关节不稳定桡骨粉碎性骨折,可有效提高术后肘关节功能,降低术后并发症。
Objective To explore the clinical effect of three different surgical methods in treatment of elbow unstable radius comminuted fracture. Methods A total of 33 patients with fresh blunt type Mason III radial head fracture in this hospital were divided into three groups,each using radial head excision,fixation or artificial radial head arthroplasty,and elbow function after surgery and long- term complications had been observed. Results Broberg and Morrey scores,carrying angle,degree of displacement and radial proximal forearm rotation in patients with artificial radial head arthroplasty were superior to other methods,and the difference was significant( P〈0. 05),but the difference in complications was not significant( P〉0. 05). Conclusion Artificial elbow radial head arthroplasty in treatment of patients with unstable radial fracture can effectively improve the postoperative elbow function and reduce the postoperative complications.
