肝门部胆管癌根治术后肝胆管缺血坏死相关性肝脓肿一例

摘要 患者 女性,58岁.因“上腹部胀痛伴皮肤巩膜黄染1周”于2013年7月15日入院.入院前1周无明显诱因出现上腹部剑突下胀痛,持续不能缓解,伴恶心、呕吐1次,为胃内容物.同时发现皮肤巩膜黄染,并进行性加重.皮肤无瘙痒,尿色深黄,大便颜色逐渐变浅.既往无上腹部手术史.体检：生命体征平稳,皮肤巩膜明显黄染,心肺未及异常,腹平软,上腹部剑突下压痛（＋）,无反跳痛、肌紧张,未及包块,Mushy征（-）,肝脾肋下未及,移动性浊音（-）.化验检查：ALT 363.5 U/L,AST 195.9 U/L,总胆红素85.6 μmol/L,直接胆红素39.1 μmol/L,白蛋白42 g/L.白细胞计数3.5×109/L,中性粒细胞百分比53.6％.
