摘要
目的调查门诊2型糖尿病药物治疗费用,探讨其影响因素。方法采集425例门诊2型糖尿病患者的基本信息,并详细记录降糖和代谢相关药物、糖尿病并发症的日均治疗费用。结果 1按糖尿病病程分为≤5年组、〉5~10年组和〉10年组,≤5年组的降糖药物、总药物日均费用均显著低于〉5~10年组和〉10年组(P值均〈0.01),〉5~10年组又显著低于〉10年组(P值均〈0.01)。2按有无并发症分为有并发症组和无并发症组。有并发症组的降糖药物、总药物日均费用均显著高于无并发症组(P值分别〈0.05、〈0.01)。3按糖化血红蛋白(HbA1c)水平分为HbA1c〉7%组和HbA1c≤7%组,HbA1c〉7%组的降糖药物、总药物日均费用均显著高于HbA1c≤7%组(P值均〈0.05)。4糖尿病教育组(33例)干预后的降糖药物、总药物日均费用较干预前有减少的趋势,但差异均无统计学意义(P值均〉0.05)。结论糖尿病的病程、HbA1c水平、并发症与药物治疗费用有关,早发现、早诊断、早治疗能有效降低糖尿病治疗费用。
Objective To investigate the medical expenditure and its influencing factors in outpatients with type 2 diabetes mellitus. Methods Clinical data of 425 type 2 diabetic outpatients were collected. The daily costs of hypoglycemic drugs, metabolic drugs and complication drugs were recorded in detail. Results (1) The patients were divided into three groups according to course of disease= group A≤5 years), group B (〉5- 10 years) and group C (〉10 years). The daily costs of hypoglycemic drugs and total costs in group A were significantly lower than those in group B and group C (all P〈0.01 ), all there were significant differences between the last two groups (both P〈0.01). (2) The patients were divided into two groups according to complications. The daily costs of hypoglycemic drugs and total costs in the patients with complications were significantly higher than those without complications (P〈0.05, P〈0.01 ). (3) The patients were divided into two groups according to the level of glycated hemoglobin A1 c (HbA1 c) .. group D (Hb1c〉 7 % ) and group E (HbA c 7 % ). The daily costs of hypoglycemic drugs and total costs in group D were significantly higher than those in group E (both P〈 0.05). (4) Health education was conducted in 33 patients. After the intervention, the daily costs of hypoglycemic drugs and total costs were decreased, but there was no statistical difference in medical expenditure before and after health education (both P〉0. 05). Conclusion The duration, HbA1c level and complications of type 2 diabetes mellitus are closely related to medical expenditure. It needs detection, diagnosis and treatment as early as possible so as to reduce medical, expenditure.
