Value of Contrast-enhanced Ultrasound in Diagnosis of Benign and Malignant Renal Tumors: A Meta-Analysis

摘要 目的系统评价超声造影对肾脏良恶性肿瘤的诊断价值。方法计算机检索PubMed、EMbase、The Cochrane Library(2013年第9期)、CBM、CNKI、WanFang Data和VIP,查找超声造影对肾脏良恶性肿瘤的诊断性试验,检索时限截至2013年9月31日。由2位评价者按照纳入与排除标准独立筛选文献、提取资料并评价质量后,采用Meta-Disc 1.4软件进行Meta分析。结果最终纳入7个研究,共462例患者。ROC曲线平面图提示呈"肩臂状"分布,Spearman相关分析提示存在明显的阈值效应(P=0.008),Meta分析结果显示SROC曲线下面积(AUC)=0.98。结论超声造影对肾脏良恶性肿瘤有较好的诊断及鉴别诊断价值。受纳入研究质量和数量限制,上述结论尚需开展更多高质量研究加以验证。 Objective To systematically review the value of contrast-enhanced ultrasound in diagnosis of benign and malignant renal tumors. Methods Such databases as PubMed, EMbase, The Cochrane Library (Issue 9, 2013), CBM, CNKI, WanFang Data and VIP were searched by Sep 31st, 2013. Two reviewers independently screened literature accord- ing to the inclusion and exclusion criteria, extracted data, and assessed the methodological quality of included studies. Then meta-analysis was performed using Meta-Disc 1.4 software. Results Seven studies involving 462 patients were finally included. ROC curve showed shoulder-arm shape. The results of Spearman suggested significance of the threshold effect. The results of meta-analysis showed that, the area under curve (AUC) was 0.98. Coudusion Contrast-enhanced ultrasound can improve diagnostic accuracy, which is of important value in the diagnosis and differential diagnosis of benign and malignant renal tumors. Due to limited quality and quantity of the included studies, the abovementioned con- clusion still needs to be verified by conducting more high quality studies.
