助力电刺激辅助步行训练对脑卒中足下垂患者下肢运动功能的影响 被引量:5

摘要 目的探讨助力电刺激辅助步行训练对脑卒中足下垂患者下肢运动功能的影响。方法选取脑卒中后足下垂患者40例,采用随机数字表法分为实验组和对照组,每组患者20例。对照组采用常规康复治疗,实验组在常规康复治疗的基础上应用助力电刺激辅助步行训练。两组患者均于治疗前后分别进行跖屈肌群肌张力,胫前肌肌力的测定,同时应用FMA对患者的下肢运动功能进行评定。结果治疗前,两组患者各项指标组间比较,差异均无显著性(P>0.05);治疗后,足跖屈群肌张力评定改善总有效率实验组75%,对照组40%,胫前肌肌力评定总有效率实验组90%,对照组55%,下肢运动功能FMA评分比较表现显著性差异(P<0.05)。结论助力电刺激辅助步行训练对脑卒中足下垂患者的下肢运动功能具有显著的提高作用,改善了患者小腿三头肌的肌张力,增强胫前肌的肌力及患者步行时的平衡能力,提高患者的日常生活能力,降低患者步行时的危险性。 【Objective】To discuss the effects of walking training assisted by electrical stimulation on motor recovery of the lower limb of stroke patients with foot drop. 【Methods】40 patients after stroke with foot drop were randomly divided into two groups, including treatment group and control group, 20 cases of patients in each group. Conventional rehabilitation and were used in control group. Meanwhile, walking training assisted by electrical stimulation was added in the treatment group. Muscle tension test of plantar flexors group and the tibialis anterior muscle strength test were used in both two groups of patients before and after the treatments. Also, motor function of lower limb were assessed by FMA.【Results】Before treatment, there were no statistically significant difference(P〉 0.05)in the two group patients; After treatment, total effective rate of improvement of plantar flexors group muscle tension were 75% in treatment group and 40% in control group. And total effective rate of improvement of the tibialis anterior muscle strength were 90% in treatment group and 55% in control group. The results of FMA states motor function of lower limb of treatment group is superior to that of control group(P〈0.05). 【Conclusions】Walking training assisted by electrical stimulation can improve lower limb movement function of stroke patients with foot drop, improve the patient's muscle tension of plantar flexors group and the tibialis anterior muscle, enhance the ability of the patients balance and activities of daily living, reduce the risk of falls when walking.
