下颌第一磨牙不同种植术非负荷期牙槽骨吸收的X线观察

摘要 目的 评价下颌第一磨牙即刻种植和延期种植非负荷期牙槽骨吸收情况.方法 用Bego柱形种植体,对24颗下颌第一磨牙按就诊类型,分为即刻种植(12枚)、延期种植(12枚)进行种植修复,术后当天和术后5~6个月时以X线法测量,对比分析两种方法非负荷期颈部牙槽骨吸收情况.结果 种植体植入后5~6个月颈部骨吸收结果,即刻种植组为(0.28 ±0.37)mm,延期种植组为(0.19 ±0.25)mm,两者无显著差异(P>0.05).结论 下颌磨牙即刻种植技术能缩短种植修复缺失牙的等待时间,一次手术完成,减轻患者痛苦,是一种理想的种植修复方式.但要求术前对种植位点的软硬组织量进行准确的估计,严格把握适应证,才能获得令人满意的临床效果. Objective To evaluate the alveolar bone absorption of the mandibular first molar in unload period of immediate implantation and delayed implantation.Methods 24 first molars were treated with Bego cylindrical implants,including 12 molars of immediate implantation and 12 molars of delayed implantation;X-ray was taken on the right day of implantation and 5 to 6 months after implantation ; a comparative study was made of the alveolar bone absorption in unloaded period between the 2 implantations.Results 5 to 6 months after implantation,the distance between crest of ridge and the top of implant in immediate group was (0.28 ± 0.37) mm,while that in delayed group was (0.19 ± 0.25) mm,there existed no obvious difference between the 2 group (P > 0.05).Conclusions Immediate implantation of mandibular molars can shorten the waiting time for implant restoration,complete the operation within one act,reduce the suffering of the patients,while accurate estimate of the soft and hard tissue at the implanting sites and strict control of indications are needed for satisfactory clinical effect.
