摘要
目的探讨莫匹罗星与庆大霉素联合治疗在预防腹膜透析相关感染疗效观察,为减少腹膜透析患者相关感染提供更好的治疗方法。方法选择2010年12月-2012年12月收治需要进行腹膜透析患者120例进行研究,按照数字随机方法将其分为观察组和对照组,每组各60例,其中观察组采用莫匹罗星与庆大霉素联合治疗,对照组采用庆大霉素进行预防性治疗,观察两组患者腹膜炎及导管出口处及隧道感染的发生,并观察两组患者治疗后鼻拭子培养结果。结果观察组腹膜炎发生率为6.67%,显著低于对照组的21.67%,两组相比,差异有统计学意义(P<0.05);且观察组导管出口处及隧道感染率分别为8.33%和5.00%,显著低于对照组的16.67%和13.33%,两组相比,差异有统计学意义(P<0.05);患者鼻拭子病原菌培养结果显示,感染患者中革兰阳性菌以金黄色葡萄球菌及表皮葡萄球菌常见,革兰阴性细菌以大肠埃希菌及鲍氏不动杆菌最为常见,且对照组细菌培养阳性率76.67%,显著高于观察组的21.67%,两组相比,差异有统计学意义(P<0.05)。结论莫匹罗星与庆大霉素联合治疗在预防腹膜透析相关感染疗效显著,明显降低了透析患者腹膜炎、导管出口处及隧道感染率。
OBJECTIVE To study the effect of mupirocin combined with gentamicin in treatment of peritoneal dialysis related infections so as to provide better ways to reduce related infections during peritoneal dialysis.METHODS Totally 120 patients with peritoneal dialysis from Dec.2010 to Dec.2012 in our hospital were studied.They were divided into two groups by method of random number table and each group had 60 cases.In the experiment group,they were treated with mupirocin combined with gentamicin.And the control group was treated with gentamicin for preventive treatment.Occurrence of peritonitis and infections at catheter exit site and tunnel were compared between the two groups,and nasal swab culture results after treatment were observed.RESULTS The peritonitis incidence was 6.67%in the observation group,which was significantly lower than 21.67%in the control group(P〈0.05).And the rate of infections at catheter exit site and tunnel was 8.33%and 5.00%in the observation group,significantly lower than 16.67%and 13.33%in the control group(P〈0.05).The nasal swab culture found that Staphylococcus aureus and Staphylococcus epidermidis were the most common gram-positive bacteria and Escherichia coli and Acinetobacter baumannii were the most common gram-negative bacteria.The positive rate of bacteria was 76.67%in the control group,significant higher than 21.67%in the observation group(P〈0.05).CONCLUSION Mupirocin combined with gentamicin had significant efficacy in treating peritoneal dialysis related infections.It significantly reduced the rate of peritonitis and infections at catheter exit site and tunnel.
