低氧诱导因子-1α与食管鳞癌关系的Meta 分析 被引量:2

Relationship between hypoxia inducible factor-1αand esophageal squamous cell carcinoma:a meta analysis

摘要 目的:探讨低氧诱导因子-1α( HIF-1α)在食管鳞癌组织中的表达及与临床病理特征的关系,为临床提供参考。方法计算机检索Cochrane Library、Pubmed、EMbase数据库以及中国生物医学文献数据库、中国学术期刊全文数据库中有关HIF-1α与食管鳞癌研究的文献,采用Stata 11.0软件包进行分析。分化程度、组织分级、T分期、淋巴结转移状况、肿瘤分期、淋巴管受侵及血管受侵采用优势比( OR)及95%可信区间( CI)进行分析。结果入选14篇文献共包括1121例食管鳞癌患者,结果显示HIF-1α在食管鳞癌组织较正常组织中的表达明显增强( OR=0.088,95% CI:0.061~0.129,P=0.000);HIF-1α与T分期、淋巴结转移明显相关(OR=0.421,95% CI:0.222~0.798,P=0.008;OR=0.387,95%CI:0.207~0.725,P=0.003),HIF-1α高表达者肿瘤浸润深度增加,更易出现淋巴结转移,肿瘤分期偏晚。而HIF-1α与食管鳞癌的分化程度、组织分级、肿瘤分期、淋巴管受侵及血管受侵均无明显相关。结论 HIF-1α蛋白高表达增加了食管鳞癌的发病风险。 HIF-1α可以作为T分期、淋巴结转移的判断指标,值得进一步深入研究。 Objective To explore the expression of hypoxia inducible factor-1α( HIF-1α) in esophageal squamous cell carcinoma and its correlation with clinicopathological features .Methods Original literatures in foreign languages regarding correlation between HIF-1αand esophageal squamous cell carcinoma were identified from Cochrane Library , PubMed, EMbase database, and Chinese original literatures were from CBM , CNKI.All analyses were performed by Stata 11.0 software.Histological grade , degree of differentiation, T stage, lymph node metastasis, tumor stage, lymphatic invasion and vascular invasion were analyzed using pooled odds ratio (OR) with 95%confidence interval (CI).Results A total of 14 studies including 1 121 patients were enrolled in this meta analysis.Comparing with normal tissue , the expression of HIF-1αin esophageal squamous cell carcinoma was significantly enhanced ( OR=0.088, 95%CI:0.061-0.129, P=0.000);HIF-1αwas significantly associated with T stage and lymph node metastasis (OR=0.421, 95% CI:0.222-0.798, P=0.008; OR=0.387, 95% CI:0.207-0.725, P=0.003). High expression of HIF-1αwas correlated with an increased depth of tumor invasion , more lymph node metastasis and advanced tumor stage , whereas there was no relation to the degree of differentiation , histological grade, tumor stage, lymphatic invasion and vascular invasion.Conclusions High expression of HIF-1αprotein correlates with an increased risk of esophageal squamous cell carcinoma.HIF-1αmay be an indicator for T stage , lymph node metastasis and tumor stage , but further studies are needed.
