EBUS-TBNA对纵隔及肺部疾病诊断的意义 被引量:3

摘要 目的探讨支气管内超声引导针吸活检术(endobronchial ultrasound-guided transbronchial needle aspiration,EBUS-TBNA)在纵隔和(或)肺门淋巴结以及肺部病变诊断中的意义。方法回顾性总结35例接受EBUSTBNA的患者的临床资料。单纯纵隔和(或)肺门淋巴结肿大11例,单纯肺部病变4例,纵隔和(或)肺门淋巴结肿大伴肺部病变20例。结果通过对35例患者进行EBUS-TBNA检查,获得65份纵隔淋巴结或肺部病变组织。32例患者明确诊断,其中恶性病变27例,良性病变5例。3例患者诊断不清,后经纵隔镜活检2例诊断为淋巴瘤,1例诊断为慢性淋巴结炎。EBUS-TBNA的敏感性、特异性、准确率、阳性预测值及阴性预测值分别为98.3%、100.0%、92.3%、100.0%和83.3%。所有患者检查耐受良好,无任何相关并发症发生。结论对于纵隔及肺部疾病的诊断,EBUS-TBNA是一种安全有效的微创活检方法。 Objective To evaluate the application of endobronchial ultrasound-guided transbronchial needle aspiration( EBUS-TBNA) in the diagnosis of mediastinal and pulmonary lesions. Methods Thirty-five patients undergoing EBUS-TBNA were enrolled in the study. Among 35 cases,11 had mediastinal and( or) hilar lymphadenectasis,4 had only pulmonary lesions and 20 had both mediastinal and( or) hilar lymphadenectasis and pulmonary lesions. Results Sixtyfive biopsy specimens were obtained. Diagnosis was confirmed in 32 patients,among whom 27 were malignant lesions and 5were benign lesions. Three patients whose diagnosis was not confirmed by EBUS-TBNA underwent further mediastinoscopy.Two cases were diagnosed as lymphoma and the remaining one was chronic lymphadenitis. The diagnostic sensitivity,specificity,accuracy,positive predictive value and negative predictive value of EBUS-TBNA were 98. 3%,100. 0%,92. 3%,100. 0%,and 83. 3%,respectively. All procedures were successful and there were no complications. Conclusion EBUS-TBNA is a safe,effective and minimally invasive biopsy method for the diagnosis of mediastinal and pulmonary diseases.
