摘要
目的:探讨减少静脉用药调配中心退药的方法,避免药品浪费及成本消耗,保证患者及时用药治疗。方法:对本院静脉用药调配中心2013年8—12月退药单进行统计及分析。结果:2013年8—12月本院静脉用药调配中心共收到输液处方420 000份,其中退药50 358份,占输液处方总量的11.99%。退药原因主要有患者病情变化、经济原因、临时决定出院、转科、死亡等导致更改医嘱或医嘱停用;医生医嘱录入错误或医生查房后未及时调整医嘱等。根据上述情况,重新调整调配中心退药规定,并制定相应措施,如临床药师每月查阅病历审核退药合理性,上报医务科进行考核;医师在退药单上注明原因等措施规范退药。结论:制定静脉用药调配中心管理措施后,有效地减少了退药事件,提高了药物治疗的安全性、有效性、合理性,避免药品浪费及成本消耗。
Objective ： To explore the Methods ： for reducing medicine - withdraw in pharmacy intravenous admixture services, in order to avoid the waste of drugs and consumption of costs, and ensure the treatment of patients. Methods ： Statistical analysis was performed to investigate medicine -withdraw prescriptions in PIVAS from August 2013 to December 2013. Results： A total of 420 000 intravenous injection prescriptions has been received in PIVAS from August 2013 to December 2013, including 50 358 medicine - withdraw prescriptions which make up 11.99% of the total. The reasons of medieime - withdraw included changing or discontinuing the doctors＇ orders due to progressing of disease, economic reasons, discharging from hospital, patient transferring to other department or death; doctors＇ orders entering error, no time for adjusting medicine orders after ward round, etc. Readjusting the rules of medicine - withdraw in PIVAS and adopting the corresponding management measures, such as the monthly rationality check of medicine - withdraw and reporting the Results ： to medical services department by clinical pharmacists, , and recording the reasons of medicine - withdraw by doctors. Conclusion ： The management measures in PIVAS was adopted to reduce the occur- rence of medicine withdrawal ,increased the safety , effectiveness and rationality of medication, and avoided the waste of drugs and consumption of costs.
