摘要 目的:探讨营养筛查工具检测营养风险及营养不足发生率和应用欧洲营养风险筛查方法(N RS2002)评估病人营养状况的可行性,提高临床护士评估病人营养状况的科学性和工作效率。方法选取2013年1月~9月我院肾内科、呼吸内科、胃肠外科、胸外科病人80例为研究对象。采用N RS2002方法对新入院病人进行营养筛查,同时根据筛查结果,护士每周与营养医师、管床医师共同依据评定检测指标,对病人营养状况进行全面评估后修改营养治疗方案。结果80例病人中营养不足发生率为21.3%,总的营养风险发生率为51.3%。老年病人组的营养风险发生率为69.6%,中青年病人组的营养风险发生率为26.5%,两组比较有显著性差异(P<0.05)。80例病人中总的营养支持率为52.5%,其中有营养风险的病人营养支持率为87.8%,肠内营养的使用比例高于肠外营养;而无营养风险的病人中有15.4%的病人接受了营养支持,营养支持方式均为肠外营养。结论 N RS2002在评价住院病人营养状况中具有较好的适用性,且简单、有效、无创、快速,可较好应用于筛查住院病人的营养状况,并为临床医生进行营养支持提供依据。由护士对病人营养支持的主动干预下,有营养风险的病人大部分可以得到营养支持,对改善病人的临床结局有利。另外,加强护士营养风险筛查培训,对改善住院病人营养状况具有积极意义。 Objective To investigate the practicability of testing the nutrition risk incidence and malnutrition incidence and using European Nutritional Risk Screening 2002(NRS2002) for inpatients'nutrition condition .To promote scienticeness and work efficiency of patients'nutrition status evaluated by nurses .Methods 80 inpatients of Nephrology ,Respiratory and cerebral surgery department were chosen as observation group and evaluate every new inpatients through NRS 2002 .Accord-ing to the result ,nurses will improve the nutrition therapeutic schedule after evaluating their nutrition condition .Result The incidence of malnutrition risk of 80 patients in observation group was 21 .3% (17/80) and total nutrition risk is 51 .3% . The incidence of nutrition risk in the old patients was 69 .6% while 26 .5% in the young patients .(P〈0 .05) Nutrition sup-port incidence of 80 inpatients was 52 .5% in which 87 .8% in nutrition risk patients .Enteral nutrition proportion is higher than parenteral nutrition proportion .15 .4% inpatients without nutrition risk take the nutrition support which is character-ized by parenteral nutrition .Conclusion NRS2002 is relatively useful ,effective for evaluating inpatients'nutrition status and it is easy to operate quickly without wound ,which is suitable to apply to inpatients'nutrition evaluation .With the action taken by nurses ,most of inpatients with nutrition risk can get nutrition support ,which is good for improving inpatients'clin-ical outcome .Besides ,strengthen the nutrition evaluation training of nurses has positive meaning for improving inpatients'nutrition condition .
