电磁影像导航系统在神经外科手术中的应用(附100例报道) 被引量:2

The usefulness of electromagnetic navigation in neurosurgery

摘要 目的探讨电磁影像导航系统在神经外科手术中的应用价值。方法应用Compass电磁影像导航系统辅助显微外科手术治疗颅内病变100例,并对导航系统在手术中定位的准确性,使用的安全性和术后治疗效果进行评估。结果导航平均注册误差(1.77±0.84)mm,术中均能准确定位病灶,系统运行良好,无导航系统相关不良事件发生。病灶全切74例,次全切17例,脓肿准确穿刺完全引流5例,血肿完全引流2例,活检2例。术后神经功能未受明显影响,无手术并发症及死亡。结论电磁影像导航系统能准确引导定位颅内病灶,有助于减少手术创伤,提高手术效果,降低死亡率、病残率,而且使用安全,可靠,是微侵袭神经外科手术的重要辅助工具。 Objective To investigate the feasibility of electromagnetic neuronavigation in routine clinical practice. Methods Electromagnetic neuronavigation was performed in 100 cases. The operational accuracy, security and reliability was evaluated. Results The surgical procedures were successful in all cases. The procedure -related morbidity and mortality rate in the series was zero. Neuronavigated image guidance was evaluated as useful in all patients. The tumors and the surrounding structures were located accurately with the mean registration error (1.77±0.84)mm. Total resection was achieved in 74 cases, and subtotal resection in 17 cases. Five cases with abscess, 2 cases with hematoma were treated with puncture and drainage, and 2 patients were treated with biopsy. The postoperative neurological functions was not significantly affected. No morbidity and mortality attributed to the use of neuronavigation system. Conclusion Electromagnetic neuronavigation is safe and reliable , accurate for surgery, and may ultimately reduce complications and improve outcome. It is helpful in minimal invasive neurosurgery.
