摘要
目的探讨颅脑疾病气管切开患者适宜的气道湿化方法。方法将颅脑疾病气管切开患者60例随机分为观察组和对照组各30例。观察组采用MR850型呼吸湿化器和文丘里装置进行气道湿化,对照组采用恒速泵持续滴入湿化法,两组湿化液均使用灭菌注射用水。结果观察组气道湿化过程中的心率及呼吸频率值较对照组更平稳(均P<0.01),血氧分压及血氧饱和度显著高于对照组(均P<0.01);观察组气道痰痂形成发生数、气管套管堵管发生数较对照组降低,气道高反应性发生数为零。结论对颅脑疾病气管切开患者使用MR850型呼吸湿化器和文丘里装置可有效调节吸氧浓度,控制气道内湿度,提高气道湿化效果,促进排痰,从而降低肺部感染发生率。
Objective To explore an optimal humidification method for tracheotomized patients with craniocerebral disease.Methods A total of 60 tracheotomized patients with craniocerebral disease were randomized in equal numbers into a control group,who received continuous humidification of the endotracheal airway by using an infusion pump to deliver sterile water,and an observation group,who received continuous humidification by using a MR850 respiratory humidifier and a Venturi device.Results The heart rates and respiration rates in the observation group were more stable than in the control group(P<0.01 for both) ; the partial pressure of oxygen and pulse oxymetry results were significantly higher,the count of sputum crust formation in the lower respiratory tract,and the count of tracheostomy tube obstruction,were lower,in the observation group than in the control group.No case in the observation group developed airway hyperreactivity.Conclusion The combination use of MR850 respiratory humidifier and Venturi device could adjust the concentration of inhaled oxygen for tracheotomized patients with craniocerebral disease,control the airway humidity,and improve humidification effect.
