摘要
目的 探讨大隐静脉移植在四肢血管损伤中的作用和肢体功能恢复情况。方法 对 6 2例应用大隐静脉移植治疗的四肢血管损伤病例的临床资料和 36例随访资料进行分析总结。结果 本组男 4 5例 ,女 17例 ,平均年龄 2 6 .3岁。交通事故伤占 4 3.5 % ,机器挤压伤占 33.9%。血管损伤部位 :小腿占 5 4 .8% ,前臂占 2 7.4 %。移植大隐静脉长度 13～ 4 5 cm。血循环重建时间 :2小时 4 5分～ 2 4小时 30分。5 9例肢体成活 ,占 95 .2 %。随访 36例 ,肢体缺血挛缩 3例 ,神经功能恢复不完全 5例 ,其余患者功能恢复满意。结论 大隐静脉移植治疗四肢血管损伤保肢成功率高 ,应作为血管移植的首选血管。
Objective To discuss the effect of the GSVT on the treatment of the extremity vascular iujuries and analyze the function recovery of the limbs.Methods The clinical data of 62 cases with extremity vascular trauma treated with the GSVT and 36 cases who were followed up were summed up and analyzed.Results The age of the 62 cases(45 male,17 female)ranged from 7 to 66 and the average age was 26.3.The vascular trauma were caused by traffic accident(43.5%) and machines(33.9%) respectively.The positions of the vascular injuries:were the leg (54.8%) and forearm(27.4%).The length of transplantation of the great saphenons vein was from 13 cm to 45 cm.The time of reconstruction of blood circulation varied from 2 hours and 45 minutes to 24 hours and 30 minutes.The GSVT were succecsful in 59 case(95.2%).Thirty six cases were followed up and 3 cases of musclar contracture,5 cases of imperfect nerve function were found.The fanction recovery in other cases were satisfied.Conclusion The great saphenons vein transplantation should be chosed firstly because the successful rate of transplantation is high and it can satisfy the demande of the limbs for their different sizes and lengths of the blood vessels.Furthemore,attention should be paid to the recovery of limbs function.
