内皮素和一氧化氮与原发性高血压左室几何形态的关系 被引量:1

摘要 目的 探讨原发性高血压 (EH)患者血浆内皮素 (ET)、一氧化氮 (NO)水平与左室几何形态的关系。方法 应用二维及三维超声研究了 82例EH患者的左室几何形态 ,同时 ,应用放免和化学法测量患者血浆ET、NO和胰岛素 (INS)水平。结果 EH患者左室几何形态 4种类型的比例为 :正常左室形态 (NG)者占 36 .59% ;向心性重构 (CCR)占 34 .1 5 % ;离心性肥厚 (ECH)及向心性肥厚 (CCH)均占 1 4 .63 %。EH亚组患者ET水平均升高 ,在CCH组ET水平最高 ;各亚组NO水平均下降。ECH组血浆INS水平最高。 Objective To investigate the left ventricular geometry patterns adapting to hypertrophy in essential hypertension(EH) and their relation to the plasma levels of endothelin and nitric oxide. Methods 82 patients with EH and 22 healthy volunteers were involved in the present study. Left ventricular geometry of EH was studied by using two- and three-dimensional echocardiography. Meanwhile, the plasma levels of endothelin (ET), insulin (INS) and nitric oxide(NO) of all subjects were determined by using radioimmunoassay and chemical method. Results (1)The frequencies of the 4 subtypes of left ventricular geometry were: normal geometry (NG) 36.59%,concentric remodeling (CCR) 34.15%, eccentric and concentric hypertrophy (ECH and CCH) both 14.63%. (2)In all subtypes of EH, the plasma level of endothelin was increased, highest in the CCH subgroup; mean while, the level of NO was all reduced. The level of INS was also elevated, being the highest in the ECH subgroup. Conclusion The levels of plasma biochemical factors are changing with the left ventricular geometry in all EH patients.
