高血压患者左房构形超声新表达指标的变化 被引量:7

摘要 目的 在探索正常人左房构形超声新表达的基础上 ,进一步观察和认识高血压患者左房构形的变化。方法 选择 45例高血压患者 (Ⅰ、Ⅱ、Ⅲ期各 15例 )和 15例正常成人 ,心电触发二维切面超声于左室收缩末期测量左房最大内径 (LAD)、面积 (LAA)和容积 (LAV) ,用体表面积标化出左房内径指数 (LADi)、面积指数 (LAAi)和容积指数 (LAV i) ,观察各期高血压患者上述六项指标的变化。结果 LAD、LAA、LAV和LADi、LAAi、LAVi在高血压患者均表现增大 ,并随着高血压分期的加重而愈加显著 ,其中高血压Ⅰ期LAD、LAA和LAV与正常对照组之间的差异无显著性 (P>0 0 5 ) ,LADi、LAAi和LAVi与正常对照组之间的差异有显著性 (P均 <0 0 5 ) ;高血压Ⅱ、Ⅲ期 6项指标与对照组之间的差异均有显著性 (其中Ⅱ期LAVP <0 0 5 ,余指标P均 <0 0 1) ;此外LADi、LAAi和LAVi在高血压各期之间的差异亦有显著性 (P均 <0 0 1)。结论 高血压各期患者左房均表现增大 ,体表面积标化的LADi、LAAi和LAVi较常规指标LAD、LAA和LAV能够更早更敏感地反映高血压患者左房构形的改变。 Objective To inverstigate the changes of the left atrial size in hypertension using a novel expressions. Methods Forty-five cases of patients with hypertension (Ⅰ,Ⅱ and Ⅲ phases, each 15 cases) and fifteen healthy adults were underwent echocardiography. The diameter, area and volume of the left atrium(LAD, LAA and LAV, respectively) were measured at the end-systolic phase of the left ventricle by ECG gating. The indices of the diameter, area and volume in relation to body surface area(LADi, LAAi and LAVi, respectively) were calculated. Results When no statistical differences in LAD, LAA and LAV of the the left atrium between the control group and the group with hypertension Ⅰ phase was shown(P>0.05 ), the novel expression LADi, LAAi and LAVi were obviously differenct between hypertension Ⅰ and the control group ( P<0.05). All six parameters are increased in hypertension Ⅱ and Ⅲ phases. Moreover, differences in LADi, LAAi and LAVi among the groups with hypertension Ⅰ, Ⅱ and Ⅲ phases (all P values <0.01 ) were significant. Between Ⅰ and Ⅲ phases only LAD,LAA and LAV have shown statistically different(P<0.01),Ⅱ and Ⅲ phases with LAV only( P<0.05 ). Conclusion The left atrium enlarged in patients with hypertension in Ⅰ,Ⅱ and Ⅲ phases. The novel echocardiographic parameters of the left atrium in relation to the body surface reflected the changes of the left atrial size more precisely and sensitively than the conventional variables.
