摘要
目的 探讨晚期肺癌血清可溶性细胞间粘附分子_1(sICAM_1)和肿瘤坏死因子_α(TNF_α)水平及其临床意义。方法 采用ELISA方法检测45例晚期肺癌患者及32名健康成人血清sICAM_1和TNF_α水平。 结果 晚期肺癌患者循环血sICAM_1和TNF_α浓度明显高于对照组 (t=19.63和6.85,P<0.001) ;Ⅲ期肺癌患者血清sICAM_1和TNF_α水平低于Ⅳ期患者 (t=3.50和2.06,P<0.01和0.05)。综合治疗后获局部控制者血清sICAM_1和TNF_α水平较治疗前明显下降 (t=5.24和2.62,P<0.001和0.05) ,治疗后4月仍保持低水平 (t=10.60和5.69 ,P<0.001) ;未获局部控制者血清sICAM_1和TNF_α水平仅轻度下降 (t=2.59和1.96,P<0.05和>0.05) ,治疗后4月复查结果与治疗前相似 (t=2.01和1.98,P>0.05)。结论 晚期肺癌患者循环血sICAM_1与TNF_α水平与肿瘤负荷及病程进展有关 ,动态检测血清sICAM_1与TNF_α水平可作为评判晚期肺癌综合治疗疗效和预后的一项客观指标。
Objective To explore the relationship between serum levels of soluble intracellular adhesion molecule_1(sICAM_1) and tumer necrosis factor_α(TNF_α) and lung cancer with late stage and its clinical significance.Methods Serum levels of sICAM_1 and TNF_α were determined by ELISA in 45 patients with lung cancer in late stage and 32 healthy adults.Results Serum levels of sICAM_1 and TNF_α in lung cancer group were significantly higher than those in normal group (P<0.01).Taking mean±2s of normal level as cut_off value,the positive rate of sICAM_1 and TNF_α in lung cancer with late stage was 97.8% and 91.1% respectively.The levels of serum sICAM_1 and TNF_α in patients with stage IV were greater than those with stageⅢ(P<0.01 and 0.05).Serum levels of sICAM_1 and TNF_α in patients without remaining tumor after treatment were decreased (P<0.001),while those were not decreased in patients with remaining tumor(P>0.05).Conclu_sion Serum concentration of TNF_α and sICAM_ 1 were related to tumor burden,consecntive measurements of TNF_α and sICAM_1 may be an useful objecitve index to evaluate prognosis and curative effects in lung cancer with late stage.
