摘要
目的 :探讨伸指腱器终腱的解剖学特点和槌状指损伤的病理机制 ,为临床治疗提供依据。方法 :对新鲜成人尸体示、中、环、小指各 2 0指的伸指腱器终腱特点进行了解剖学测量 ,并对终腱的滑动距离进行了动态观察和测量。结果 :各指伸指腱器终腱的形态结构大体相似 ,但是中指 (17.12± 3 .3 )mm和环指 (16.3 3± 2 .9)mm的长度明显长于示指 (14 .73± 4.1)mm和小指 (10 .91± 2 .5 )mm ,具有显著性差异 (P<0 .0 1) ,并测出了各指终腱薄弱区的位置 5 .5~ 8.6mm ;同时检测了终腱运动功能与外力的关系 ,当近侧指间关节 (PIP)伸直时终腱的滑动距离较PIP屈曲时减少 40 %以上 ,表明终腱对外力的缓冲能力也减少了40 %。结论 :伸指腱器终腱的薄弱区是断裂的易发部位 ,也是功能重建的关键部位 ;终腱的形态结构。
Objective:To discuss the morphological charactrastics of the terminal extensor apparatus and the pathomechanics of mallet finger to provide morphological and mechanical basis for the treatment of mallet finger.Methods:The structure and sliding distance of the terminal extensor apparatuses in index,middle,ring and little fingers were observed and measured on 20 fresh cadeveric upper extremity speciments.Results:The structure of terminal extensor apparatus was similar in every finger,the length of terminal extensor apparatus in the middle (17.12±3.3 mm) and ring finger (16.33±2.9 mm) was longer than those in index(14.73±4.1 mm) and little finger(10.91±2.5 mm)( P <0.01);the weak area of the terminal apparatus was 5.5~8.6 mm;the sliding distance of terminal apparatus decreased 40% while extension than flexion,which showed the ability of compensation for external forces decreased 40% in the terminal extensor apparatus.Conclusions:The weak area in the terminal apparatus is the position of rupture,which is also the key position for reconstruction;the three key causes of mallet finger are structure of the terminal apparatus,movement function and external force.
