摘要
目的 :观察 β 受体阻滞剂治疗慢性心力衰竭 (心衰 )对心脏自主神经功能的影响。 方法 :心衰组 2 2例 ,口服美托洛尔3个月 ,对照组心功能正常者 6 0例 ,所有患者均进行 2 4hHolter检查 ,对美托洛尔治疗前后及对照组进行心率功率谱时域和频域分析。结果 :心衰组治疗前心率变异性 (HRV)值较对照组显著降低 ,其中时域指标 :2 4h正常R R间期标准差 (SDNN)、2 4h连续相邻R R间期差值均数的标准差 (rMSSD)显著降低 (P <0 .0 0 1) ;频域指标 :总频谱 (TF)、低频 (LF)和高频 (HF)亦明显降低 (P <0 .0 0 1)。在 β 受体阻滞剂治疗 3个月后复查 ,心衰组HRV值均得到改善 (P <0 .0 5 )。 结论 :β 受体阻滞剂可改善慢性心衰患者的心率变异性。
Objective:To study the effect of β blocker on heart rate variability in chronic congestive heart failure(CHF).Methods:Twenty two patients with CHF were treated with metoprolol for three months,sixty patients with normal cardiac function acted as controls.The heart rate power spectral density and time domain were analyzed with 24 hour electrocardiographic recordings before and after using metoprolol.Results:Compared with the control group,time domain analysis in CHF group before the treatment revealed a significant decrease in SDNN\,rMSSD( P <0.001) and frequency domain analysis revealed a marked decrease of total\,low and high frequency components( P <0.001).All values improved in CHF group three months after therapy with metoprolol.Conclusions:β blocker can significantly improve the heart rate variability in CHF.
