摘要
目的 :探讨治疗下肢静脉曲张的最佳术式。方法 :术前标出静脉曲张部位 ,常规行顺行大隐静脉高位结扎 ,剥脱大隐静脉主干 ;继之点式抽剥法剥脱小腿曲张静脉团 ,术后抬高患肢 2 0°～ 30° ,穿循环驱动袜 1～ 3个月。继发性下肢静脉曲张均在治疗原发病后实施手术。结果 :6 1例患者术后无 1例发生切口感染及皮下血肿 ,均于术后 10～ 12天拆线 ,4 5例得到随访 ,2例复发 ,小腿外观无明显手术瘢痕。结论 :点式抽剥法治疗下肢静脉曲张具有手术创伤小、操作简便易行、切口并发症低、术后恢复快。
Objective:To explore the optimal surgical method for lower extremities varicosis.Methods:The sites of varicosis were marked preoperatively.The greater saphenous vein was ligated and the main channel of the greater saphenous vein stripped out,then the mass of varicose veins of the legs was stripped out by punctate stripping intraoperatively.The extremities were elevated 20° to 30° and the patients were required to wear medical compression stockings for 1 to 3 months.The secondary lower extremities voricosis should be operated on after the primary diseases were treated.Results:No cut infection or subdermal hematoma were found in all the 61 cases postoperatively;the stitches were taken out on 10 to 12th day after operation.Forty five cases were followed up and local recurrence found in 2 cases.There were no obvious scars on the legs.Conclusions:For treatment of lower extremities varicosis,punctate stripping has the advantage of minor injury,easy performance,lower complications of the wound,quick recovery and leaving small scars.
