B超预测Ⅲ级胎盘功能的可行性研究 被引量:4

摘要 目的探讨B超诊断Ⅲ级胎盘功能的可能性。方法制定Ⅲ级胎盘超声分级诊断标准 ,将Ⅲ级胎盘划分为ⅢA 和ⅢB 两个亚级。观察各亚级胎盘的组织显微结构形态变化 ,采用体视学方法进行组织计量学分析。分别测定两个亚级的血E3 、血hpL ,监测羊水指数、胎心监护及胎儿预后以了解和判断胎盘功能。结果两组胎盘显微结构形态学上有明显变化 ,组织计量学分析发现血管合体膜数、合体结节数、纤维素样坏死、绒毛间隙距离、绒毛血管直径等指标两组胎盘差异有显著统计学意义 (P <0 0 1)。在血E3 、血hpL值、羊水指数 (AFI)、无负荷试验 (NST)、震荡声音刺激试验 (VAST)及新生儿窒息率等方面差异有显著统计学意义 (P <0 0 1)。结论ⅢA 亚级胎盘和ⅢB 亚级胎盘在组织学改变和胎盘功能等方面均有明显差异 ,ⅢB 亚级能够反映胎盘功能的储备能力和代偿能力。因此 ,Ⅲ级胎盘超声亚分级对预测Ⅲ级胎盘的功能状况是可行的。 Objective To explore the value of ultrasound subgroups of grade III placent in estimating the placental function. Methods Grade III placental was divided into IIIA and IIIB subgroups according to B-type ultrasound. The microstructure were observed and tissue metrology analyses were made in these two subgroups. Serum E3, hpL, amniotic fluid index (AFI), and fetal heart sounds were surveyed. Results There are obvious differences in placental tissue microstructures between the two groups, such as the number of zoarium membrane and zoarium nodus in blood vessel, fibrotic necrosis, distance of tomentum gap, diameter of tomental blood vessel( P <0.01). there are also obvious defferences in serum E3, hpL AFI, non-load test vibration sound stimulating test , and the asphyxia rate of fetus ( P <0.01). Conclusion There are marked differences in histological stuctures and functions between IIIA and IIIB placent, ultrasound subgrouping play an important role in prognosising the function state of grade III placent.
