非体外循环冠状动脉旁路移植术与神经元损伤的观察

摘要 目的对比观察体外循环与非体外循环冠状动脉旁路移植术 (CABG)对神经元的损伤情况。方法 17例CABG患者随机分为两组 :非体外循环组 (实验组 ) 8例 ,手术在常温心脏跳动下进行 ;体外循环组 (对照组 ) 9例 ,术中阻断升主动脉 ,鼻咽温度为 2 5℃~ 2 7℃。动态监测围术期所有患者颈内静脉血中S 10 0 β蛋白和神经元特异性烯醇酶 (NSE)的水平变化。 结果两组患者术中及术后早期 ,其S 10 0 β蛋白和NSE水平明显高于术前 (P <0 0 5或P <0 0 1) ,其中S 10 0 β蛋白术后 3d恢复至术前水平 ,NSE术后 1d恢复至术前水平 ;实验组S 10 0 β蛋白从术中到术后 1d明显低于对照组 (P <0 0 5或P <0 0 1) ,NSE从手术毕到术后 8h明显低于对照组 (P <0 0 1)。结论 (1)颈内静脉血中S 10 0 β蛋白和NSE的水平变化 ,可作为评价CABG对神经元损伤程度的可靠指标 ;(2 ) Objective To investigate the effects of off-pump coronary artery bypass grafting(CABG) on neuronal injury. Methods 16 CABG patients were divided into off-pump group(n=8) and on-pump group(n=8). The parameters of protein S-100β and neuron-specific enolase(NSE) in neck venous blood were measured during the perioperative period. Results The changes of the protein S-100βin off-pump group were significantly lower during operation to 1 day postoperation than those in on-pump group( P <0.05 or P <0.01), and the NSE were significantly lower on operation finished to 8 hours postoperation than those in on-pump group( P <0.01); The changes in the both groups during and after operation were significantly higher than before operation( P <0.05 or P <0.01). Conclusions 1. The analysis of protein S-100β and NSE might as a index of the underlying neurophychiatric deficits of neuronal injury. 2. The neurophychiatric deficits of neuronal injury may be more adversely affected on-pump CABG than off-pump.
