摘要
目的 了解原发性肝癌患者介入治疗前的生命质量和术后变化情况。资料与方法 采用万崇华等编制的肝癌特异性生命质量评定量表对广东省 3家医院 175例经介入治疗的原发性肝癌患者进行生命质量评定。调查时间为介入术前、术后 1个月及 3个月 ,对 3个时点的生命质量总分及各维度得分进行统计分析。结果 生命质量总分平均值在介入术后出现明显提高 ,术前、术后 1个月及 3个月的分值分别为 :115 .4± 45 .1、12 8.1± 2 8.3、12 9.8± 46 .4(P <0 .0 1) ;维度分析示术后 1个月时 ,生理、心理、症状 3个维度得分明显上升 ,社会维度得分则下降 ,有显著差异 (P <0 .0 1) ;术后 3个月时 ,心理维度与术前无差异 ,而生理、症状、社会维度则与术前仍有显著差异(P <0 .0 5 )。结论 总体上介入治疗能够提高中、晚期肝癌患者的生命质量。提高程度与患者肝癌分期、肝功分级和介入治疗方法等密切相关。
Objective To evaluate the quality of life (QOL) in the patients with hepatocellular carcinoma (HCC) both before and after interventional therapy (IVT).Materials and Methods A QOL questionnaire specially for HCC patients (compiled by WAN Chonghua, et al.) was used to investigate 175 HCC patients, by face to face method, from 3 hospitals in Guangdong province who had received interventional therapy. The investigation time was pre-IVT and 1, 3 months post-IVT. The data obtained at the above three points of time were analyzed.Results The total score obtained at pre-IVT and 1, 3 months post-IVT was 115.4±45.1, 128.1±28.3 and 129.8±46.4, respectively (P<0.01). Domains analysis showed that physiology, psychology and symptom score were markedly increased, and the social score was decreased (P<0.05) at 1 month post-IVT. But at 3 months post-IVT the psychology score was resumed.Conclusion Generally, the QOL of HCC patients show a marked improvement after IVT, which is closely related with the tumor stage, liver function and interventional management. It is hopeful that QOL may be regarded as a new index in evaluating the effectiveness of the interventional therapy.
