颈内动脉狭窄及闭塞的超声影像学评价 被引量:18

摘要 目的 彩超 (CDI)、经颅多普勒超声 (TCD)对颈内动脉 (ICA)狭窄 (≥ 70 % )或闭塞诊断及颅内外血流动力学与DSA比较。方法 经全脑数字减影血管造影 (DSA)检查证实的ICA狭窄或闭塞患者 86例行CDI和TCD常规检测 ,确定ICA狭窄或闭塞的形态学和颅内血流动力学及侧支循环的开放情况 ,与DSA对比分析。结果 ①与DSA比较颅外ICA检测结果的敏感性为 98.4% ,特异性 96.0 % ,准确率 97.6% ;②与DSA比较TCD对前交通动脉 (ACOA )判断的敏感性 98.4% ,特异性 95 .0 % ;后交通动脉 (PCOA)敏感性 97.5 % ,特异性 10 0 % ;眼动脉 (OA)开放的敏感性 86.9% ,特异性 71.4% ;③患侧Vmca(62 .7± 2 4.3 )cm/s较对侧 (12 0 .3± 45 .4)cm/s明显减低 (P <0 .0 1) ;患侧大脑中动脉 (MCA)的PI值 (0 .76±0 .2 1)较对侧 (1.0 7± 0 .2 1)明显减低 (P <0 .0 1)。 Objective To compare the hemodynamic changes of color Doppler imaging (CDI) and transcranial Doppler (TCD) with digital subtraction angiography (DSA) in patients with severe stenosis (≥70%) or occlusion of internal carotid artery (ICA).Methods Eighty six cases of ICA stenosis or occlusion confirmed by DSA were examined by using CDI and TCD, and hemodynamic changes were evaluated.Results ①Compared with DSA,CDI and TCD in detecting stenosis or occlusion ICA, the overall accuracy (OA) was 97.6%, the sensitivity (SEN) 98.4%, the specificity (SPE) 96.0%. ②Compared with DSA, ACOA detection in TCD had SEN of 98.4% and SPE of 95.0%; PCOA detection in TCD had SEN of 97.5% and SPE of 100%;directional changes in OA had SEN of 96.9% and SPE of 71.4%. ③Vmca on the occlusion sides was lower than the contralateral sides (62.7±24.3)cm/s vs (120.3±45.4)cm/s, and the same with PI. Conclusion The combination of CDI and TCD can offer us a noninvasive quantifications of the inner and outer cerebral blood flow and assessments of collateral supply of ICA stenosis or occlusion.
