摘要
目的 探讨自然组织谐波成像 (NTHI)对心脏瓣膜厚度测量值的影响。方法 分别用常规超声基波成像 (FI)、NTHI1(探头频率 1.8~ 3 .6MHz)和NTHI2 (探头频率 2 .1~ 4.2MHz)测量 2 5例风湿性二尖瓣狭窄患者二尖瓣厚度值 ,并与病理解剖厚度值 (PAM )进行比较。结果 FI、NTHI1、NTHI2测值和PAM ,水平轴分别为 (3 .3 0± 0 .97)mm、(3 .93± 1.0 3 )mm、(3 .82± 0 .99)mm和 (3 .2 0± 1.0 6)mm ;垂直轴分别为 (4 .2 3± 1.41)mm、(4 .63± 1.5 8)mm、(4 .60± 1.60 )mm和 (4 .3 9± 2 .3 0 )mm。经统计学检验 ,NTHI测值高于FI和PAM ,水平轴、NTHI1及图像质量未改善组更显著。NTHI1(X1)和NTHI2 (X2 )测值与PAM (Y )呈高度相关 ,回归方程 :水平轴Y =0 .92 9X1,Y =0 .965X2 ;垂直轴Y =0 .93 7X1,Y =0 .943X2 。结论 NTHI增大了瓣膜厚度测量值 ,且在水平轴方向、低频发射及图像质量未改善组更明显。
Objective To assess the effect of native tissue harmonic imaging (NTHI) on increasing mitral valve leaflets thickness and to find the relation between thickness measurements with NTHI and patho anatomic measurements (PAM). Methods Twenty five consecutive patients (12 male and 13 female) with rheumatic mitral stenosis (RMS) were studied. The valve thickness was measured in fundamental imaging(FI)?NTHI1( 1.8 ~ 3.6 MHz) and NTHI2( 2.1 ~ 4.2 MHz) modes respectively. PAM was got after the valve replacement operation with slide gage. According to the scores of endocardial border visualization, all patients were divided into two groups: improved group and unimproved group. Results Near horizontal axis, the thickness measurements were ( 3.30 ± 0.97 )mm,( 3.93 ± 1.03 )mm,( 3.82 ± 0.99 )mm and ( 3.20 ± 1.06 )mm in FI,NTHI1,NTHI2 and PAM respectively; near vertical axis, the mean thickness were ( 4.23 ± 1.41 )mm,( 4.63 ± 1.58 )mm,( 4.60 ± 1.60 )mm and ( 4.39 ± 2.30 )mm in FI,NTHI1,NTHI2 and PAM. The mean thickness measurements of mitral valves with NTHI was greater than those with FI and PAM, especially with NTHI1 in the horizontal axis and in the unimproved group. A good correlation was found between the mean measurements of mitral valve thickness with NTHI1(X 1),NTHI2(X 2) and PAM(Y). The regression equation was Y= 0.929 X 1,Y= 0.965 X 2 near horizontal axis; Y= 0.937 X 1,Y= 0.943 X 2 near vertical axis. Conclusions NTHI overestimates mitral valve leaflets thickness with a more exaggerated increase,especially in the horizontal axis,with the lower transmitting frequency and in the unimproved group.
