人颞肌的肌构筑学研究及临床意义

摘要 目的 查清人颞肌的肌构筑学特征 ,了解颞肌前、后两部的生物力学属性 ,阐明其生理功能。方法 对人 10侧颞肌的肌构筑指数进行了测算和量化分析。结果 颞肌前部为多羽肌 ,内部有两个与肌表面平行的腱板 ,肌纤维排列较垂直 ,从浅深两面附着于腱板上 ,后部肌纤维较长 ,呈前后水平走向附着于腱膜上。前、后两部各构筑指数比较 ,除生理横切面积外 ,均有显著性差异。颞肌后部肌重大于前部 ,但生理横切面积与前部比较无显著性差异。相反 ,前部肌的生理横切面积 /肌重比率却明显高于后部肌 ,后部肌的肌纤维长 /生理横切面积比率高于前部肌。前、后两部各项构筑指数在左右侧比较均无显著性差异。结论 颞肌前部属力量型肌 ,后部较前部倾向于速度型肌。前部肌在维持下颌位置和产生咬合力上起主要作用 。 Objective To find out the architectural features of human temporalis muscle and the properties of biomechanics in both anterior and posterio portions of the muscle,and to elucidate its physiogical function?Methods The muscle architecture parameters were measured for ten human temporalis muscles and analyzed by a mechanical formula?Results The results showed that the anterior portion of temporalis muscle was a multipennate muscle with two aponeurosis arranged in parallel with the surface of the muscle , and its muscle fibers vertically arranged ,attaching to the aponeurosis from both superficial and deep portions .The muscle fibers of posterior portion were more length , going horizontally forward and attaching to the aponeurosis.The weight of the posterior portion was obviously more than that of the anterior, but there existed no statistical differences for physiological cross sectional area.On the Contrary , the ratio of physiological cross sectional area/muscle weight in the anterior portion was significantly higher than that in the posterior.The ratio of muscle fiber length/muscle weight in the posterior , however , was remarkably higher than that in the anterior. There were not any statistical differences for the architectural indices in both the anterior and posterior between the left and right laterals of temporalis muscle.Conclusions These results indicated that the anterior portion of temporalis muscle is designed for tension production , but the posterior for velocity produciton , of which the anterior plays a major part in maintaining mandibular position and producing occlusive force , but the posterior does a critical role in mandibular retrusion and latero tursion.
