摘要
目的 探讨彩色多普勒超声在胃底贲门静脉曲张的表现和超声诊断价值。方法 用二维灰阶、彩色多普勒超声对胃底贲门静脉曲张做观察分析。结果 113例肝硬化伴有门静脉高压患者接受了超声检查。超声显示胃底贲门静脉曲张 46例 ,超声假阴性病例 2 5例。超声诊断正确率 :77.9% (88/113 ) ;超声诊断敏感性 64 .8% (4 6/71) ;特异性 :10 0 %(4 2 /4 2 )。频谱多普勒测定的静脉血流为 (15 .95± 2 .78)cm/s。曲张的静脉直径 0 .8～ 1.4cm。结论 胃充盈下的超声检查能显示扩张的静脉的范围和程度 ;二维彩色多普勒和频谱多普勒能帮助了解病变血流的流速。肝脏左叶萎缩 ,胃底位置偏高、扩张的静脉轻微是假阴性的主要原因。
Objective To explore the characteristics of cardia fundus varicosity with ultrasonography and evaluate the application of ultrasonographic diagnosis to cardia fundus varicosity. Methods Cardia fundus varicosity was observed and analyzed by two dimensional grey scale ultrasonography, color Doppler imaging (CDI) and pulsed Doppler. Five cases of cardia fundus varicosity were performed with three dimensional CDE (3D CDE). Results One hundred and thirteen patients with hepatocirrhosis complicated with portal hypertension were examined by ultrasonography. Forty six of them were diagnosed with cardia fundus varicosity using this method, and 25 cases were false negative. The accuracy rate was 77.9% (88/113), and the sensitivity was 64.8% (46/71), the specificity was 100% (42/42). The blood flow in veins measured was (15.95±2.78)cm/s. The dilated vein in diameter is 0.8-1.4cm. Conclusion The variciform area and severity could be identified by ultrasonography after stomach filled. The speed of blood flow could be measured by CDI and pulsed Doppler. The main reasons for false negative may be related with atrophy of left liver, fundus elevating, mild varicosity.
