摘要
目的 探讨胰腺损伤的诊断方法和治疗原则。方法 回顾性分析了 1 950～ 2 0 0 0年收治的 65例胰腺损伤病人的临床资料 ,总结了胰腺损伤的诊断和处理方法 ,并对术后并发症进行了分析。结果 胰腺损伤中 86 .1 5 %是多发伤和腹部多脏器损伤 ,病情复杂 ,术前诊断困难 ,1 0 0 %开放性胰腺损伤和 72 5 %闭合性胰腺损伤是经开腹探查证实 ,其治疗是针对胰管损伤的治疗。结论 对于开放性胰腺损伤、有明显腹部症状的闭合性胰腺损伤、CT检查疑有Ⅰb、Ⅰc、Ⅱ型胰腺损伤均应及时的开腹探查 ,根据胰腺损伤的程度选择合理的术式 。
Objective To investigate the principle of the diagnosis and treatment for pancreatic trauma. Methods Retrospective analysis on the 65 patients who suffered from pancreatic trauma between 1950 and 2000. Results 86.15% of these 65 patients combined with multiple trauma and other abdominal organ trauma. Preoperative diagnosis was difficult. All the open and 72.5% closed pancreatic trauma were diagnosis by exploratory operation. The different main pancreatic duct injury decided different surgical management. Conclusion The indications for exploratory operation should be considered when open pancreatic trauma,closed pancreatic trauma combined with obvious symptoms, type Ib, Ic, Ⅱ suspected by CT examination were determined. The rational selecting operative procedures for the extent of the pancreatic trauma to induce the postoperative complications.
