摘要
目的 研究面神经监测在听神经瘤手术中保护面神经的方法和意义。方法 将62例患者随机分为术中面神经监测(IFNM)组(32例)与非IFNM组(30例),然后比较分析两组患者术前、术后10d的面神经功能状况。结果 术后10d面神经功能Ⅰ~Ⅲ级者两组分别为26例和15例,Ⅳ~Ⅴ级者分别为6例和15例;随访后两组面神经功能Ⅰ~Ⅲ级者分别为29例和20例,Ⅳ~Ⅴ级者分别为3例和10例;两组比较差异显著。结论 面神经监测能显著提高听神经瘤术中面神经的解剖和功能保存率,提高患者生存质量。
Objective To explore the method and significance of facial nerve preservation during the acoustic neuroma surgery by using intraoperative facial nerve monitoring (IFNM). Methods All 62 patients were randomly divided into the IFNM group (32 cases) and the control group (30 cases). Facial nerve function (FNF) assessed by a modified House-Brackmann grading (H&B) before the operation, ten days after the surgery was compared between the two groups. Results On the tenth days after the surgery there were 26 patients with FNF of HB grade Ⅰ -Ⅲ and 6 patients with FNF of HB group Ⅳ-Ⅴ in IFNM group, but there were 15 patients with FNF of HB grade Ⅰ - Ⅲ and 15 patients with FNF of HB gradeⅣ - Ⅴ in the control group. During the follow up, there were 29 patients with FNF of HB grade Ⅰ - Ⅲ and 3 patients with FNF of HB grade Ⅳ - Ⅴ in IFNM group, but there were 20 patients with FNF of HB grade Ⅰ - Ⅲand 10 patients with FNF of HB grade Ⅳ-Ⅴ in the control group. There was significant difference in the FNF between both the groups. Conclusions The rates of anatomic and functional facial nerve preservation could be significantly increased by IFNM. The prognosis is obviously improved by IFNM in the patients with acoustic neuromas.
