摘要
目的 :探讨非体外循环冠状动脉搭桥术的优点、适应征及移植材料的选择。方法 :手术在全麻常温下进行 ,正中切口。例 1搭桥 2支 ,例 2搭桥 3支。结果 :例 1,手术时间 2 .5h ,带气管插管时间 4 .5h ,例 2 ,手术时间 3h ,带气管插管时间 4h ,均在术后 9d痊愈出院。结论 :非体外循环下的冠状动脉搭桥术安全可行 ,拓宽了搭桥术的指征。且可减少手术创伤与并发症 ,降低医疗费用 ,在移植材料上 ,首选动脉材料。
Objective: To report the results of 2 cases operated on off-pump coronary artery bypass grafting(OPCAB) in our hospital and to investigate the advantages and the indication for operation and the choice of type of bypass graft of OPCAB. Methods: in July 2001 two patients underwent OPCAB. Access was through a standard middle sternotomy incision. The patient 1 had two vessels bypass, while the patient 2 had three vessels bypass. Results: For the patient 1, duration of operation was 2.5 hours with 4.5 hours of assisted respiratory. The hospitalization cost was 23000 yuan RMB. The patient 2 was operated of for 3 hours with 4 hours of assisted respiratory and spent 25000 yuans RMB. Two patients were discharged at 9 days after operation. Conclusion: OPCAB is an alternative treatment of coronary disease and has less hospitalization cost than that of CCABG has. The artery is first recommended in the choice of bypass graft.
