Significance of fatty acid synthase expression in non-small cell lung cancer

摘要 目的 探讨脂肪酸合成酶 (FAS)在非小细胞肺癌 (NSCLC)中的表达及其临床意义。方法 采用免疫组织化学S P技术 ,对 175例NSCLC手术标本进行FAS的检测 ,分析其与多种临床因素的关系。结果 FAS在NSCLC中的阳性表达率为 31.4% (5 5 /175 )。在腺癌中的阳性表达率(2 2 .4% )明显低于非腺癌 (38.4% ;P =0 .0 36 ) ;有血管受侵或骨转移的患者 ,其FAS阳性率 (75 .0 %和46 .9% )明显高于无血管受侵或没有骨转移的患者 (2 9.3%和 2 8.0 % )。FAS的表达与患者年龄、性别、吸烟指数、肿瘤大小、淋巴结转移、肿瘤分期、分化程度、有无局部复发和远处转移等临床因素无相关性 (P >0 .0 5 )。虽然FAS阳性和阴性患者的生存率差异无显著性 (P =0 .0 6 6 ) ,但是分层分析显示 ,FAS阳性的Ⅰ期患者预后较阴性的患者差 (P =0 .0 0 5 )。结论 FAS与NSCLC大部分临床指标没有相关性 ,但在其早期有提示预后的作用。 Objective To evaluate the expression of fatty acid synthase (FAS) in non-small cell lung cancer (NSCLC). Methods FAS was examined by immunohistochemical S-P technique in 175 specimens of NSCLC patients. Multiple clinical factors were analyzed according to their relation with expression of FAS. Results The overall FAS expression rate was 31.4% (55/175). The expression of FAS in the non-adenocarcinoma patients was significantly higher than that of adenocarcinoma patients (38.4% vs 22.4%, P =0.036). Higher FAS expression was also detected in patients who had vascular invasion or bone metastasis than those without ( 75.0% vs 29.3%, P =0.02 and 46.9% vs 28.0%, P =0.037). But, there was no significant difference between FAS and other clinical factors such as age, sex, smoking index, tumor size, stage, degree of differentiation, lymphatic metastasis, local recurrence or distant metastasis. Although there was no significant difference in the survival rates of FAS positive and negative patients ( P =0.066), the survival rate of FAS positive stage Ⅰ patients was lower than that of negative ones ( P =0.005). Conclusion Fatty acid synthase in the specimens of non-small cell lung cancer patients has no correlation with most clinical factors, except that, in early lesions, it may signify poor prognosis.
