摘要
【目的】 探讨儿童注意缺陷多动障碍 (ADHD)的诊断方法。 【方法】 对家长或教师认为“多动”的 6~11岁儿童 198例 ,采用DSM Ⅳ的标准、持续性注意测验 (CAT)和Conners父母症状问卷 (PSQ)分别作诊断。 【结果】 3种方法诊断为ADHD的分别有 110例、10 5例和 10 3例 ,相应占 5 5 .6%、5 3 .0 %和 5 2 .0 %。后二者分别与前者比较差异均无显著性 (χ2 分别为 0 .2 5和 0 .5 0 ,P均 >0 .0 5 )。CAT和PSQ与DSM Ⅳ诊断比较 ,分别有 96.2 %和 97.1%为符合与基本符合。CAT的敏感性、特异性和准确性分别为 91.8%、95 .5 %和 93 .4% ,PSQ的三项相应为 90 .9%、94.3 %和 92 .4%。 【结论】 CAT和PSQ的敏感性、特异性和准确性分别相近或高于目前最常用的Conners简明症状问卷的 76.0 %、92 .9%和82 .1% ,对DSM Ⅳ诊断标准具有较客观的补充作用 ,值得试用以至推广应用。
To explore the diagnostic method of attention deficit hyperactivity disorder(ADHD) in children. One hundred and ninty eight cases of 'hyperactivity child' considered by their parent or teacher,age from 6 to 11 years,were examined with DSM Ⅳ,continuous attention task(CAT) and parent symptom questionnaire(PSQ). One hundred and ten cases(55.6%),one hundred and five cases(53.0%) and one hundred and three cases (52.0%) were diagnosed as ADHD by the three methods separately.It is not significantly different between the former and the two latters separately(χ 2=0.25,0.50; P >0.05).96.2% and 97.1% scoring of CAT and PSQ are in correspondence with the diagnosis by DSM Ⅳ separately.The sensitivity,specificity and accuracy of CAT are 91.8%,95.5% and 93.4%;that of PSQ are 90.0%,94.3% and 92.4%. [Conclusion] The sensitivity,specificity and accuracy of CAT and PSQ are better than or same as that of ASQ(76.0%,92.9% and 82.1%;compared with CAT,χ 2=7.39,0.29 and 8.22; P <0.05,>0.05 and <0.05;compared with PSQ,χ 2=6.44, 0.08 and 5.10, P <0.05,>0.05 and <0.05) which are used frequently at the present.CAT and PSQ can be complement of DSM Ⅳ and be worthy of recommendation.
