摘要
目的 探讨脊髓血管胚胎发育对脊髓动静脉畸形治疗的指导意义。方法 采用各胎龄胚胎及胎儿、新生儿和成人尸体标本 ,观察脊髓血管。回顾 32 4例脊髓血管畸形的诊治。结果 描述对脊髓血管胚胎各期的发育特征 ,分析脊髓纵裂畸形和椎体血管瘤伴发的脊髓动静脉畸形病例。在髓内动静脉畸形的手术中发现畸形团与正常脊髓实质之间有一层增生的胶状质相隔。结论 脊髓血管最早发生在脊髓表面 ,逐渐长入或被卷入脊髓实质。脊髓动静脉畸形最早发生在胎龄 3周以前。大部分脊髓动静脉畸形团可以切除 ,而不损伤脊髓。
Objective To describe the characters of fetal spinal vascular structure, discuss the etiology and treatment strategy of spinal arteriovenous malformations.Methods Observing the vascular system in cadaver spinal cord samples of human embryo, fetus, newborn and adult. Reviewing the treatment of 324 cases of spinal vascular malformations. Results Capillary has been found in the surrounding matrix before the neural canal closure. With the growth of the cord, the superficial capillary gives off perforators to enter the cord. The spinal vessel has had adult morphologic character since 3rd fetus month. Some typical spinal vascular malformations have been analyzed. Conclusions Spinal arteriovenous malformations are supposed to originate before 3rd fetal week. There is connective tissue barrier between the neural tissue and vascular nidus, which suggests the malformation can be resected surgically.
