摘要
目的 :为预防和治疗质粒介导的超广谱 β-内酰胺酶 (ESBL)细菌感染提供依据。方法 :采用 E-test法对可疑细菌进行 ESBL 检测 ,用 K- B法做药物敏感试验 ,用 WHONET4进行分析。结果 :在 890株革兰阴性杆菌中检出 ESBL 44株 ,其中大肠埃希菌 18株、肺炎克雷伯菌 10株、阴沟肠杆菌 9株、费劳地枸橼酸杆菌 3株、嗜水气单胞菌 2株、鼠伤寒沙门菌 1株、液化沙雷菌 1株 ,对亚胺培南、呋喃妥因、阿米卡星、环丙沙星的敏感率分别为 10 0 %、6 8.2 %、2 9.5 %、2 5 .0 %。结论 :产 ESBL 细菌分布广泛、阳性率高 ,易借助耐药质粒传播 ,具有较高的交叉耐药性和多重耐药性。
Objective:To provide evidence for preventing and treating the extended-spectrum beta-lactamase(ESBL) produing strains which were transferred by plasmid.Methods:E-test was adapted to detect the ESBL of the suspicius isolates.Microbial sensitivity tests was carried on with K-B,and analysed it with WHONET 4.Results:In 890 strains Gram-negative bacilli,44ESBL productions were detected,18 strains of which were Escherichia coli,10 were Klebsiella pneumoniae,9 were Enterobacter cloacae,3 were Citrobacter frun-dii,2 were Aeromonas hydrophils,1 was Salmonella typhimurium,1 was Serrattia liguefaciens,The sensitive rate of ESBL to imipenem,nitrofurantoin,amikacion,ciprofloxacin was100%,68.2%,29.5%,25.0%.Conclusion:ESBL production distribute broad, are detected positive frequently and spread easily by resistant plasmid. It's cross resistance and multi-resistance are high.
