地氟醚低流量洗入紧闭环路麻醉的摄取规律

The uptake of desflurane during closed-circuit anesthesia with low initial fresh-gas-flow

摘要 目的 研究地氟醚高浓度、低流量洗入时的摄取规律。方法 择期全麻下行上腹部手术的ASA I-Ⅱ级患者80例,随机分为A、B两组,洗入期:A组新鲜气流量为:氧气0.7 L/min;B组新鲜气流量为:氧气0.4 L/min,氧化亚氮 0.3 L/min。两组Tec-6地氟醚蒸发器刻度均为18%。维持期:两组新鲜气流量均不变,蒸发器刻度均改为8%。结果 A组平均洗入时间为(4.56±0.68)min,B组平均洗入时间为(4.05±0.52)min,二者无统计学差异(P>0.05)。两组间各时点 FA/FI均无显著差异(P>0.05)。结论 低流量洗入地氟醚、氧化亚氮时,氧化亚氮并不影响地氟醚的体内摄取过程。地氟醚低流量洗入紧闭环路麻醉安全可行。 Objective To investigate the uptake of desflurane during closed-circuit anesthesia with low initial fresh-gas-flow(FGF) instead of conventional high initial FGF. Methods Eighty ASA Ⅰ- Ⅱpatients (45 male, 35 female) undergoing upper abdominal surgery were randomly allocated to one of two equal groups (n=40) : group A and B. Premedication consisted of midazolam 0.07mg·kg-1 im, 30min prior to surgery. Before induction of anesthesia the patients were preoxygenated with O2 6L/min for 3 min to wash out nitrogen. Anesthesia was induced with intravenous midazolam 0.12 mg·kg-1 , fentanyl 3μg· kg-1 and vecuronium 0.1 mg·kg-1 . After tracheal intubation the patients were mechanically ventilated to maintain PaO2 at 4.5-5.0 kPa. In group A FGF was O2 0. 7L· min-1 whereas in group B O2 0.4 L· min-1 + N2O0.3 L ·min-1 during desflurane wash-in and desflurane vaporizer (Tec-6)dial was set at 18% . During maintenance of anesthesia Tec-6 was set at 8% but FGF was maintained unchanged. The mean time of desflurane wash-in, the inspired concentration (Fi) and alveolar concentrations (Fa) of desflurane were recorded.Results The mean time of desflurane wash-in was (4.56 ± 0.68) min in group A and (4.05 ±0.52)min in groups B. There was no significant difference in Fa/Fi between the two groups at any time during anesthesia ( P > 0.05 ). Conclusions With low initial FGF addition of N2O does not affect the uptake of desflurane. The low-flow desflurane anesthesia is safe and economical.
