摘要
目的 研究犬单肺移植术后的血液动力学变化以及低温的影响。方法 健康犬 14条随机分为供肺犬和受肺犬各 7条。受肺犬行单肺移植 30分钟后 ,采用物理方法 (腹腔大网膜及颈部、四肢大血管处用冰袋冷敷 )降温至 35℃ ,观察 30分钟 ,然后复温。测定受肺犬的血液动力学指标 :心输出量 (CO)、心率 (HR)、平均动脉压 (MAP)、平均肺动脉压 (MPAP)、肺毛细血管楔压(PCWP)、中心静脉压 (CVP)、肺血管阻力 (PVR)、体循环血管阻力 (SVR)、左室每搏功 (LVSW )和右室每搏功 (RVSW)。结果 与术前基础值比较 ,单肺移植后 30分钟的MPAP、PVR、SVR和RVSW显著升高 ,CO和LVSW显著降低 (P <0 0 5 )。降温后 ,PVR和SVR比肺移值后 30分钟进一步升高 ,HR、MAP、CO、LVSW进一步降低 (P <0 0 5 ) ,MPAP和RVSW降温前后保持不变。复温后上述各指标均恢复至基础值。结论 单肺移植可引起犬肺动脉压升高和体循环功能紊乱 。
Objective To investigate the changes of hemodynamics in canine underwent single-lung transplantation (SLT).Methods Fourteen healthy canines were randomly assigned to donor canines (n=7) and recipient canines (n=7).Hypothermia of 35℃ for 30min in the recipient canines following left SLT was induced with physical methods,and then the animals were warmed to normal temperature.The parameters of hemodynamics were measured.Results Compared with the baselines,mean pulmonary arterial pressure (MPAP),pulmonary vascular resistance (PVR),systemic vascular resistance (SVR),and right ventricular stroke work (RVSW) were significantly increased,while cardiac output (CO),and left ventricular stroke work (LVSW)were significantly reduced (P<0.05) at 30min following SLT.During hypothermia,PVR and SVR were progressively increased,while heart rate (HR),mean arterial pressure (MAP),CO, and LVSW were lower than those at 30min after SLT (P<0.05),and MPAP and RVSW remained unchanged.After rewarming,all of above parameters returned to baselines.Conclusion Hemodynamics become unstable after SLT,and may be deteriorated by hypothermia.
