摘要
目的 比较全麻下骨科手术自体血回输及异体输血对病人血浆白介素 2 (IL 2 )及白介素 6 (IL 6 )的影响。方法 5 7例骨科病人 ,随机分为两组 :Ⅰ组 ,自体血回输 ,31例 ;Ⅱ组 ,异体输血 ,2 6例。两组均于麻醉前、术后第 2天、第 7天采外周静脉血 3ml,肝素抗凝离心后取血浆冷冻备检测。结果 术后第 2天 ,Ⅱ组IL 2较术前及Ⅰ组明显下降 (P <0 0 5 ) ;IL 6在术后第 2天、第 7天两组病人都有升高 ;术后第 2天Ⅰ组较术前和Ⅱ组升高非常显著 (P <0 0 1) ,术后第 7天Ⅰ组较术前和Ⅱ组升高显著 (P <0 0 5 )。结论 自体血回输组病人IL 2下降不明显 ,IL 6明显升高 ,对病人的免疫功能影响较小 ;异体输血组IL 2明显下降 ,IL 6升高不显著 。
Objective To investigate the effects of autogeneic or allogeneic transfusion on plasma IL-2 and IL-6 in osteopathic patients under general anesthesia.Methods 57 patients scheduled for operation were randomly divided into autogeneic group (group Ⅰ,n=31) and allogeneic transfusion (group Ⅱ,n=26).Plasma IL-2 and IL-6 were measured before anesthesia,on the 2nd and 7th day after operation.Results On the 2nd day after operation,IL-2 of group Ⅱ decreased more significantly than that of group Ⅰ and that of before operation (P<0.05).IL-6 was increased significantly in both groups on the 2nd and 7th day.On the 2nd day,IL-6 in group Ⅰ was increased significantly than that before operation and that in group Ⅱ (P<0.01).On the 7th day,IL-6 in group Ⅰ was higher than that before operation and that in group Ⅱ (P<0.05) Conclusion In the patients receiving autologous transfusion,the plasma IL-2 is not markedly decreased,but IL-6 is increased significantly.In those receiving allogeneic transfusion,however,plasma IL-2 is decreased significantly and IL-6 is not. The results indicate that,compared with allogeneic transfusion,autogeneic transfusion has little effect on immune function.
