摘要
目的 为了提高前列腺增生症 (BPH)患者开放手术的治疗效果。方法 从1995年7月至2000年7月我院对158例良性前列腺增生症患者行改良Madigan术 ,对中叶增生突入膀胱≥3cm者采用联合膀胱颈“ +”切开 ,以切除过多的膀胱颈后唇粘膜。结果 158例手术一次成功 ,术后恢复快 ,出院时测得最大尿流率14～32ml/s(平均16.4ml/s) ,国际前列腺症状 (IPSS)评分(6.8±2.3)分 ,生活质量(QOL)评分(1.5±0.8)分。随访6个月～5年 ,未发现严重的并发症 ,疗效满意。结论 该术式既保留了完整的尿道粘膜及膀胱颈 ,又可以通过联合膀胱颈“ +”切开 ,切除重度增生突入膀胱内的前列腺中叶 ,取得较好的疗效 ,是较为理想的前列腺增生的开放性手术方法。
Objective To evaluate the effects of an open operation on hyperplasia of prostate?Method From July 1995 to July 2000,modified Madigan operation was performed on one hundred and fifty -eight cases of benign hyperplasia of prostate in our hospiral?In order to excise the excessive posterior-lip mucosa from the neck of bladder,combined '+'incision on the neck of bladder were adopted on patients whose middle lobe projected into bladder?Result All operations were successful.Patients had a quick recovery from operation.The urinary output rate measured was ranged from 14 ml/L to 32ml/L(average 14.6ml/L)?IPSS was o.8±0.3 . QOL was1.5±0.8.In a period(ranged from 6 months to 5 year) of following up ,no severe compliction were found and the effects was satisfied?Conclusion The modified Madigan operation preserved urethral mucosa and the neck of bladder,but excised the middle lobe of prostote that projected into bladder through the '+'incision on the neck of bladder. It is an adoptadle operation for hyperplasia of prostote?
