摘要
目的 提高局部进展期胰腺癌的治疗效果。方法 5 9例Ⅱ、Ⅲ期胰腺癌 ,采用三维适形放射治疗 (3DCRT)结合区域性动脉灌注化疗 (综合组 ) 33例 ,单独应用 3DCRT(对照组 ) 2 6例。结果 疼痛缓解率综合组和对照组分别为 78.8%和 80 .8%。综合组和对照组有效率 (CR +PR)分别为78.8%和 4 2 .3% (P <0 .0 1)。两组 1、2、3年生存率分别为 72 .2 %、4 8.5 %、9.1%和 5 0 .0 % ,15 .4 %、3.9% ,2年生存率综合组优于对照组 (P <0 .0 1)。结论 对不能切除的胰腺癌 ,行 3DCRT与区域性动脉化疗结合 。
Objective To improve the therapeutic response and survival of locally advanced pancreatic cancer treated with radiochemotherapy. Methods Fifty nine patients with stage Ⅱ Ⅲ pancreatic cancer were divided into two groups: 33 patients were treated with three dimensional conformal radiotherapy (3DCRT) plus regional intra arterial infusion chemotherapy (CT)and 26 patients received 3DCRT alone.Results The response (pain alleviating) rates were 78.8% in 3DCRT+CT group and 80.8% in 3DCRT group,while the overall response (CR+PR) rates were 78.8% and 42.3% (P<0.01), respectively. The 1 ,2 ,3 year survival rates were 72.2%,48.5%,9.1% and 50.0%,15.4%,3.9%,with the difference in the 2 year survival between these two groups statistically significant (P<0.01).Conclusions Three dimensional conformal radiation therapy combined with regional intra arterial infusion chemotherapy is an effective a the treatment for unresectable locally advanced pancreatic cancer.
