摘要
目的 研究胃腺癌及其相应淋巴结转移灶中上皮性钙粘素 (E -cd)的表达及其意义。方法 应用免疫组织化学S -P法检测 2 0例正常胃粘膜、2 0例异型增生、10 9例胃腺癌原发灶及相应淋巴结转移灶中E -cd。结果 E -cd在异型增生、胃腺癌原发灶及淋巴结转移灶中阳性表达率分别为 85 % (17/ 2 0 )、4 0 .37% (44 / 10 9)和2 3.85 % (2 6 / 10 9) ,三者之间有显著差异 (P <0 .0 1)。E -cd表达与胃腺Lauren分型、TNM分期和预后显著相关。E -cd的下降主要见于弥漫型胃癌及其在淋巴结转移灶中的表达明显低于原发灶。结论 E -cd可反映胃腺癌的生物学特性 ,是预测患者预后有价值的指标。
Objectives To study the expression and prognostic value of E-cadherin in gastric carcinoma and their lymph-node metastases.Methods Immunohistochemical method was used to detect E-cd expression in 20 normal gastric mucosa tissues,20 gastric epithelial dysplasia,109 advanced gastric adenocarinomas and matched lymph-node metastases.Results The positive expression rates of E-cd in gastric epithelial dysplasia,gastric adenocarcinomas and matched lymph-node metastases were 85% (17/20), 40.37%(44/109),and 23.85%(26/109) respectively with significant difference(P<0.01).The positive expression of E-cd was closely related to Lauren type,TNM categories,and prognosis.The expression of E-cd was closely linked to diffuse-type gastric adenocarcinomas.Comparing the primary cancers with the matched lymph-node metastases,the expression rate of E-cd was reduced.Conclusions E-cd might be valuable marker in assessing biological behavior and prognosis in gastric adenocarcinomas.
